fix(sdk): run Finally hooks on every stage outcome and close spans for refused answers - #980
Proposal item 1.2: a stage's Did hooks are skipped when the stage fails, and there was no hook that always runs.
A plugin that opens something in Will and closes it in Did (a timing span, a timer, a lock) left it open on every
failure. Two related gaps made it worse:
When the finalize stage of tools/call failed on the success path (the tool's output schema refusing its answer,
or a non-finite number), the runner rethrew the error without recording it: state.flowError stayed unset and Did('finalize') was skipped, so no hook could see the failure.
@frontmcp/observability closed the tools/call span and recorded the request-log failure in Did('finalize'), so
for a refused answer the span stayed open forever and the request log said ok.
This PR adds a Finally hook type and keeps Did success-only:
Finally(stage) is available everywhere Will / Did / Around are: FlowHooksOf() and every pre-built export
built on it (ToolHook, ResourceHook, PromptHook, ListToolsHook, HttpHook, JobHook, ...).
A stage's Finally hooks run after its Will hooks, body (with Around) and Did hooks on every outcome: success,
respond, fail, abort, next, handled, a Will hook that failed or responded, an Around hook that failed.
When the stage failed (fail, an abort or a thrown error), the runner records the error in state.flowError before the
stage's Finally hooks run. For fail(error) (a hook's ctx.fail(), a tool's this.fail()) that is error
itself, no longer the control-flow signal that carried it, whose message was empty. state.flowError keeps the
run's first failure: a later stage's failure never replaces it. A finalize failure on the success path is now
recorded too; the client still gets exactly the same error.
A Finally hook cannot change the outcome: if it responds, fails or throws, the runner logs a warning naming the
stage and goes on; the other Finally hooks still run, in priority order.
tools:call-tool declares flowError in its state, so ctx.state.flowError is typed there (as it already was on resources:read-resource, prompts:get-prompt and jobs:execute-job).
@frontmcp/observability closes flow spans and records request-log failures in Finally hooks of the finalize
stage instead of Did hooks: finalize (tool, resource, prompt, agent, HTTP, skills, completion, elicitation,
subscribe, set-level) and cleanup (legacy SSE, streamable HTTP and stateless HTTP transports).
The read-resource half of 1.2 (finalize replacing the original error) was already fixed by #967 and is untouched.
Not in this PR: the list, auth, OAuth, skills-HTTP and well-known flows have no finalize stage, so observability ends
their spans in a Did hook of their last stage, which a failure (and, in auth:verify / session:verify, an early
answer) skips. Closing those needs a stage that always runs in each of those flows, a separate change.
User-facing change
What changed: new Finally hook decorator; state.flowError is set when finalize fails; observability closes
spans and records failures for answers refused by the output schema.
Before → after:
ToolHook.Finally(...) / FlowHooksOf(name).Finally(...): did not exist → runs after the stage on every outcome
and reads ctx.state.flowError; its responses and throws are ignored with a warning, and the values it set on the flow state before a respond, fail or throw the runner drops are put back (a job's answer, for one; an object it changed in place is not)
(Ignored a throw from a Finally hook of '<stageKey>': it cannot change the stage's outcome).
Did hooks: unchanged, still run only when their stage succeeded; now documented as such.
A tool answer refused by its output schema: state.flowError unset, no hook saw it → state.flowError is the InvalidOutputError for the Finally('finalize') hooks. The client still gets the same INVALID_OUTPUT error.
state.flowError after ctx.fail(error) or a tool's this.fail(error): the FlowControl signal, with an empty message → error itself. The observability plugin and the job retry check already unwrapped the signal, so
their output is unchanged; a job's run record now keeps the real error.
state.flowError after a hook's FlowControl.abort(reason): unset, so the stage's Finally hooks (observability
included) took it as a success → an InternalMcpError reading Flow aborted: <reason>. A job attempt a hook
aborts keeps that error in its run record (was: The job attempt ended before it produced a result).
Observability for that call: tools/call span never ended, request log status: ok with no error → span ended
with status ERROR and the exception event, request log status: error with code: INVALID_OUTPUT and the
client's error id.
ObservabilityPlugin hook methods _httpDidFinalize, _toolDidFinalize, _resourceDidFinalize, _promptDidFinalize, _agentDidFinalize are renamed _…FinallyFinalize, and _sseDidCleanup, _shttpDidCleanup, _slhttpDidCleanup are renamed _…FinallyCleanup (internal, underscore-prefixed).
frontmcp.dev pages affected (if known): the plugin hooks guide (hook decorator types, @Did vs @Finally), the
decorators reference (hook decorators), the observability tracing / request-log pages (a refused answer is recorded).
Skills catalog entries updated:
frontmcp-development/references/create-plugin-hooks.md: five decorators, what ctx.state.flowError holds, a "@Did runs on success, @Finally
always runs" section with an example, options/priority, tool-level hooks, common patterns, checklist,
troubleshooting.

frontmcp-development/SKILL.md: the hooks row of the routing table.
frontmcp-observability/examples/telemetry-api/plugin-telemetry.md (and its manifest entry): dropped the stale

Tests
Unit (libs/sdk):
new hooks/__tests__/flow-hooks.finally.spec.ts: Finally after success (after Did, lower priority first),
after a body failure, after a Will hook fails, after a Will hook calls fail() (they read the error it was
given), after a Will hook responds, after an Around hook fails, on a finalize refusal (while Did('finalize') is skipped); a Finally hook that responds or throws changes neither a
successful answer, nor a failed call's error, nor an INVALID_OUTPUT refusal, later Finally hooks still run, and
each ignored throw is logged as a warning naming the stage.
flows/__tests__/flow-error-state.spec.ts: the output schema's refusal is state.flowError for Finally('finalize'), and Did('finalize') skips it; when finalize fails after a failed call, state.flowError
stays the call's own failure, the one the client gets.
flows/__tests__/flow-stage-hook-lists.spec.ts: plugin and run-added (tool-class) Finally hooks run in stage
order.
hooks/__tests__/flow-hooks.behaviour.spec.ts: Finally hooks run after Did hooks, lower priority first.
tool/flows/__tests__/call-tool.output-schema.spec.ts: both refusals (schema and non-finite number) are recorded
for Finally('finalize'), and the client still gets INVALID_OUTPUT.
Unit (libs/observability):
server-telemetry.spec.ts on a real createFetchHandler() server: a refused answer ends the tools/call span
with ERROR and logs INVALID_OUTPUT with the client's error id (it failed before the fix: no tools/call span
finished).
observability-plugin.spec.ts: every hook of the plugin on a finalize stage (finalize, cleanup) is a Finally hook.
E2E:
demo-e2e-hooks/e2e/finally-hooks.e2e.spec.ts: a finally-recorder plugin records how execute and finalize
ended for refused-answer (InvalidOutputError on finalize), failing-execute (PublicMcpError on both) and audited-tool (ok); two other Finally hooks try to answer and to fail the call, and the client still gets INVALID_OUTPUT, The ledger is unreachable and the tool's own answer; the ignored throw is in the server log.
demo-e2e-standalone/e2e/observability-refused-answer.e2e.spec.ts: with observability configured, a refused
answer's tools/call span ends as error and its request log records INVALID_OUTPUT (before: span open,
request log ok).
demo-e2e-dts-consumer: hooks-must-compile.ts uses ToolHook. Finally and FlowHooksOf(...).Finally and reads ctx.state.flowError against the built declarations.
Ran:
nx run-many -t lint test build for the affected libraries (sdk, observability, skills, plugins, the plugin-*
packages, adapters, testing, cli, edge, nx-plugin, react, demo): all green, no new lint warnings in the changed
files (sdk: 7210 tests, observability: 498, skills: 249).
nx test demo-e2e-hooks (17), demo-e2e-standalone (51), demo-e2e-dts-consumer (7), plus demo-e2e-jobs (43)
and demo-e2e-errors (14) for the runner's failure paths: all green.
tsc --noEmit on the sdk and observability lib and spec tsconfigs and on the changed e2e apps; prettier on every
changed file; node scripts/fix-unused-imports.mjs origin/release/1.10.x (no change).
The new unit and e2e specs were run red first: ToolHook. Finally is not a function, and the observability e2e
reported the refused answer's tools/call span open and its request log ok. The fail() test read an empty
message before state.flowError held the error itself; the first-failure test failed with the guard removed;
the plugin test listed the three transport Did('cleanup') hooks.
Summary by CodeRabbit
New Features
Added Finally hooks that run after a stage regardless of success or failure and can inspect its error. Their responses and exceptions do not change the operation’s outcome.
Made failure details available to tool-call hooks.
Bug Fixes
Improved reporting of failed operations, including invalid tool outputs, in traces and request logs.
Preserved hook-provided reasons in failed job records.
Documentation
Updated hook guides and examples to explain Finally behavior and how it differs from Did hooks.
A stage's Did hooks are skipped when it fails, so a plugin that opened something in
Will and closed it in Did leaked it on every failure. Finally(stage) is now available
on FlowHooksOf() and every hook export built on it: its hooks run after the stage's
Will, body and Did hooks on every outcome, read the failure from state.flowError, and
cannot change the outcome (what one responds or throws is logged and ignored).
The runner records a stage's failure before its Finally hooks run and keeps the run's
first failure, so a finalize failure on the success path (an output schema refusing
the answer) is now recorded while the client still gets the same error.
tools:call-tool declares flowError in its state.
@frontmcp/observability closes flow spans and records request-log failures in
Finally('finalize') hooks, so a refused answer ends the tools/call span as failed and
logs INVALID_OUTPUT instead of leaving the span open.
…ose transport spans in Finally hooks
- state.flowError held the FlowControl signal of a fail(error), whose message is empty, so a Finally hook
could not read why the stage failed; it now holds the error itself.
- Test that state.flowError keeps the run's first failure when the finalize stage fails after it.
- Observability ends the legacy SSE, streamable HTTP and stateless HTTP spans in Finally('cleanup') hooks,
the finalize stage of those flows, as it does for every other flow's finalize stage.
- Catalog: what ctx.state.flowError holds, and that a Will hook's respond() skips the stage's Did hooks.
- One-line comments in the new e2e fixtures.

Exclude finally hook keys from plan stages.
If a flow plan contains finallyExecute, parseTypedKey treats it as a base stage. The runner then runs the Finally('execute') hook as that stage’s body, in addition to running it after execute. On the second invocation, a thrown error can change the flow outcome instead of being ignored. Recognize finally in parseTypedKey, as the parser already does for the other typed hook keys. The public flow plan accepts stage-name arrays, and the runner uses this parser to select executable stages. (raw.githubusercontent.com)

Outside-diff finding on flow.instance.ts (parseTypedKey and finally keys): valid, fixed in 73f71a9. parseTypedKey now reads a finally<Stage> key as the Finally hook key of <stage>, the same way it reads will/did/around keys, so a plan that lists finallyExecute no longer runs the Finally hooks as a stage body. Covered by libs/sdk/src/flows/__tests__/flow-typed-keys.spec.ts.
Merge-risk note (a custom stage whose name begins with finally is read as a Finally hook key and not run as a stage): no change. parseTypedKey already reserves the will, did and around prefixes this way on release/1.10.x. finally joins that set, which is what lets a plan list finallyExecute without running it as a stage body. None of the 140 @Stage names in the repo starts with any of the four prefixes. finalize doesn't match, because the check is for the whole word finally.
Docstring coverage warning: no change. The new code follows the stack's rule of adding no comments unless one is necessary, and its behaviour is covered by the specs and the catalog entries listed in the description.

Nitpick on flow-hooks.finally.spec.ts (a throwing Did('execute') hook): added in ce34ed2. A filtered Did('execute') hook throws did hook broke for post_did_broke after the tool's body has succeeded. The test checks that both Finally hooks of execute read that error from state.flowError, that the finalize hooks still run, and that the client gets did hook broke. It passed without code changes, so the behaviour was already right and only the test was missing.
ce34ed2 also follows 71c3a29, which makes the finally-hooks.e2e.spec.ts log check poll for the warning line (bounded at 5 s) instead of reading the server's logs once.

Record abort before running Finally hooks.

Record abort before running Finally hooks.
FlowControl.abort is an error control used by tool hooks. runOneStage currently records only fail and unknown_error. An abort therefore leaves state.flowError unset. The observability Finally hooks then take their success branch and can close spans as successful instead of recording the abort.

Record transport cleanup failures in the request log.
The three transport cleanup Finally hooks call only onTransportDidFinalize. That function closes the span as failed when state.flowError is set. None of the hooks calls recordRequestLogFailure. If a cleanup hook fails, the request log for that request can end with status: 'ok' and no error, while the span ends with error status. The PR description states that transport cleanup failures are recorded in the request log, so this gap is inside the PR's own scope.
A Will, Did, or Aroundcleanup hook that throws reaches state.flowError before the Finally hooks run. The runner then rethrows the error at the unsuppressed finalize stage. Add the same request-log call to each transport cleanup Finally hook:

…hooks
runOneStage recorded only fail and unknown errors, so after a
FlowControl.abort the Finally hooks saw no failure and observability closed
the span as a success. Every failed outcome is now recorded; an abort reads
as "Flow aborted: <reason>".
Follow-up on flow.instance.ts:617-623 (FlowControl.abort and state.flowError): valid, fixed in 683c37f. runOneStage now records every failed outcome, using the same FAILED_OUTCOMES set the Around chain uses (fail, abort, unknown_error). An aborted stage therefore sets state.flowError before its Finally hooks run, and the observability hooks close the span as an error instead of a success.
An abort's FlowControl has an empty message (the reason sits in output), so it's recorded as an InternalMcpError reading Flow aborted: <reason>, which gives a Finally hook or a span something to say. A fail() still records the error it was given. The new test in flow-hooks.finally.spec.ts failed before the fix (the hooks saw ok) and passes now. The catalog's create-plugin-hooks.md describes the abort case.

A Finally hook that replaced a job's answer in state and then threw an Error
kept the replacement, which the job recorded and returned. The runner now
restores the values set before any throw it drops, not only before a
respond() or fail(). The catalog says objects changed in place are not.

the snapshot are removed rather than set to undefined; ensure deletion through
the state proxy reaches the stored data, preserving true key absence after a

- Line 61: Update the `@Finally` documentation to distinguish hooks that
respond, fail, or throw from hooks that return normally: normal returns preserve
`ctx.state` assignments. Document that on `ExecuteJobFlow`, an `execute` or
`validateOutput` hook assigning `state.answer` does so before `updateRunState`
and `finalize`, so the reassigned answer is persisted and returned.

… throw
The restore set them to undefined, so they still read as present. The flow
state proxy now passes delete through to its data, and the restore deletes
them. Catalog: a Finally hook that returns normally keeps its state writes.

Preserve custom flow stages that begin with finally.

Preserve custom flow stages that begin with finally.
FlowPlan accepts arbitrary string stage names. stageBasesOf now removes finallyCheck from every plan group because parseTypedKey classifies it as a finally hook key. The registered finallyCheck stage body is therefore skipped. Before this change, finallyCheck remained a base stage and ran.

Review comment at @libs/sdk/src/flows/flow.instance.ts around lines 91 - 97:
Update parseTypedKey and its use by stageBasesOf so a custom stage named
finallyCheck remains a base stage and its body runs, while genuine finally hook
keys are still classified as hooks. Use an established distinction or stage-name
precedence to resolve the ambiguity; do not classify every key beginning with
“finally” as a hook.

…ts group has no <something> stage
Since Finally hooks, a plan stage such as finallyCheck was read as the Finally
hook key of check and silently skipped, though it ran before. It is read as a
hook key only when the same plan group lists check.
Follow-up on flow.instance.ts:91-97 (custom stages that begin with finally): valid, fixed in a6484d5, and this corrects my earlier note on the same point. The will/did/around prefixes were reserved before this PR, but finally is new, so a custom plan stage such as finallyCheck, which ran on 1.10.15, would have been skipped silently.
stageBasesOf now reads finally<Stage> as the Finally hook key of <stage> only when the same plan group also lists <stage>. finallyExecute next to execute is therefore still never run as a stage body (the earlier outside-diff fix), and a lone finallyCheck runs as the stage it was. will/did/around keys are read as before. Tests in flow-typed-keys.spec.ts: ['parse', 'finallyCheck'] gives ['parse', 'finallyCheck'], and ['execute', 'finallyExecute', 'willExecute'] gives ['execute'].

What and why
Proposal item 1.2: a stage's
Didhooks are skipped when the stage fails, and there was no hook that always runs.A plugin that opens something in
Willand closes it inDid(a timing span, a timer, a lock) left it open on everyfailure. Two related gaps made it worse:
finalizestage oftools/callfailed on the success path (the tool's output schema refusing its answer,or a non-finite number), the runner rethrew the error without recording it:
state.flowErrorstayed unset andDid('finalize')was skipped, so no hook could see the failure.@frontmcp/observabilityclosed thetools/callspan and recorded the request-log failure inDid('finalize'), sofor a refused answer the span stayed open forever and the request log said
ok.This PR adds a
Finallyhook type and keepsDidsuccess-only:Finally(stage)is available everywhereWill/Did/Aroundare:FlowHooksOf()and every pre-built exportbuilt on it (
ToolHook,ResourceHook,PromptHook,ListToolsHook,HttpHook,JobHook, ...).Finallyhooks run after itsWillhooks, body (withAround) andDidhooks on every outcome: success, respond, fail, abort, next, handled, a
Willhook that failed or responded, anAroundhook that failed.fail, an abort or a thrown error), the runner records the error instate.flowErrorbefore thestage's
Finallyhooks run. Forfail(error)(a hook'sctx.fail(), a tool'sthis.fail()) that iserroritself, no longer the control-flow signal that carried it, whose
messagewas empty.state.flowErrorkeeps therun's first failure: a later stage's failure never replaces it. A
finalizefailure on the success path is nowrecorded too; the client still gets exactly the same error.
Finallyhook cannot change the outcome: if it responds, fails or throws, the runner logs a warning naming thestage and goes on; the other
Finallyhooks still run, in priority order.tools:call-tooldeclaresflowErrorin its state, soctx.state.flowErroris typed there (as it already was onresources:read-resource,prompts:get-promptandjobs:execute-job).@frontmcp/observabilitycloses flow spans and records request-log failures inFinallyhooks of the finalizestage instead of
Didhooks:finalize(tool, resource, prompt, agent, HTTP, skills, completion, elicitation, subscribe, set-level) and
cleanup(legacy SSE, streamable HTTP and stateless HTTP transports). The read-resource half of 1.2 (finalize replacing the original error) was already fixed by #967 and is untouched.
Not in this PR: the list, auth, OAuth, skills-HTTP and well-known flows have no finalize stage, so observability ends
their spans in a
Didhook of their last stage, which a failure (and, inauth:verify/session:verify, an earlyanswer) skips. Closing those needs a stage that always runs in each of those flows, a separate change.
User-facing change
Finallyhook decorator;state.flowErroris set whenfinalizefails; observability closesspans and records failures for answers refused by the output schema.
ToolHook.Finally(...)/FlowHooksOf(name).Finally(...): did not exist → runs after the stage on every outcomeand reads
ctx.state.flowError; its responses and throws are ignored with a warning, and the values it set on the flow state before a respond, fail or throw the runner drops are put back (a job's answer, for one; an object it changed in place is not)(
Ignored a throw from a Finally hook of '<stageKey>': it cannot change the stage's outcome).Didhooks: unchanged, still run only when their stage succeeded; now documented as such.state.flowErrorunset, no hook saw it →state.flowErroris theInvalidOutputErrorfor theFinally('finalize')hooks. The client still gets the sameINVALID_OUTPUTerror.state.flowErrorafterctx.fail(error)or a tool'sthis.fail(error): theFlowControlsignal, with an emptymessage→erroritself. The observability plugin and the job retry check already unwrapped the signal, sotheir output is unchanged; a job's run record now keeps the real error.
state.flowErrorafter a hook'sFlowControl.abort(reason): unset, so the stage'sFinallyhooks (observabilityincluded) took it as a success → an
