Rebrand: Customer Voice survey #1031
Description
Part of #1024. Step 7, ship together with 1.0.1.
- Update the survey header, title and question wording to the new name.
- Keep the satisfaction question comparable with earlier responses (wording change only, same scale).
- Match the in-product survey card wording from the UI strings issue.

Done. The live Customer Voice survey now uses the new name: welcome text ("the DocumentDB extension for Visual Studio Code"), Q1 ("Overall, how satisfied are you with the DocumentDB extension?", matching the in-product survey card), thank-you sign-off and mailto subject, and footer. Q1 keeps the same 5-star scale and labels, so CSAT stays comparable with earlier responses. The survey and project were renamed to "DocumentDB extension customer survey" and "DocumentDB extension surveys".
