Syncs the following org-standard workflow stub(s) from combsmith/.github (standards/workflows/), deployed verbatim:

  • initiative-driver.yml
  • pr-review.yml

Opened by scripts/deploy-standard-workflows.sh. Stubs are thin callers; all behaviour lives in the reusables. See standards/ci-standards.md. Labeled standards-sync and left for the normal review/auto-merge pipeline — the deploy script never merges directly.

Summary by CodeRabbit

  • Chores
    • Initiative dispatch now observes the rate-limit gate’s defer decision; gate errors or missing decisions do not prevent dispatch.
    • The pull request review workflow now uses an updated stable version. Its permissions and inputs remain unchanged.

Problem

See the linked issue.

Risk

Medium — changes GitHub Actions workflow behavior, which is exercised only post-merge; verify via the affected workflow runs.

Test plan

No test files were added or updated. Verification: bash scripts/dev-lead-lint.sh (shellcheck --severity=warning) ran pre-commit; the existing CI (bats + lint) guards the change.

Rollback

Revert this PR. No non-revertible side effects (no tags, migrations, or external state).

Monitoring

Watch the affected workflow run(s) in the Actions tab and this PR's Lint check for regressions.

Dev-Lead — fix-reviews (no-changes)

Agent reasoning
- **Review threads:** none open. The two reviews (`don-petry` and `coderabbitai[bot]`) are both `COMMENTED`, which is neutral, and neither is a change request.
- **Failing check:** the only failure is `autofix`. It ran prettier on `.github/workflows/pr-review.yml`, committed the result, and the push was rejected: "refusing to allow a GitHub App to create or update workflow `.github/workflows/pr-review.yml` without `workflows` permission". That is a token-permission limit in CI, not a defect in the code.
- **Why I left it:** `pr-review.yml` is an org-standard stub synced verbatim from `combsmith/.github`, and this PR is labeled `standards-sync`. Reformatting the stub would drift it from the template, and the next sync would revert it. If the formatting is wrong, the template in `combsmith/.github` is where to change it.
- **Other checks:** the tests, coverage, CodeQL, SonarCloud, gitleaks and the dependency audit all passed.
- **Issue comments:** I did not run the Phase 1b issue-comment enumeration. The Phase 0 data showed no stale dispositions, and the harness reported none.

Summary

Both stubs are byte-identical to the combsmith/.github standards/workflows source of truth. All action and tooling refs are pinned to immutable SHAs I verified via the API, and the only injection-relevant change removes an inline ${{ github.repository }} template expansion, which is an improvement. No exploitable vulnerability was found. Escalating anyway because the two continue-on-error steps trigger the CI-weakening hard-stop. They are an intentional fail-open around a throttling gate, not a security control. Two more items need a human: the org PAT is handed to cross-repo code, and the autofix check is red (prettier drift in pr-review.yml that it cannot push).

Downstream impact

This change is consumed by 5 downstream repo(s) that pin the affected reusable workflow / lib / prompt. Impacted consumers:

Impacted shared surfaces:
  - .github/workflows/pr-review.yml

Impacted consumers (5, fetching up to 10):
  - combsmith/.github-private (pins .github/workflows/pr-review.yml)
      .github/workflows/pr-review-canary.yml
      .github/workflows/pr-review-trigger.yml
      .github/workflows/test-dev-lead.yml

      .github/workflows/pr-review.yml
  - combsmith/markets (pins .github/workflows/pr-review.yml)
      .github/workflows/agent-ingress.yml

Findings

  • major: continue-on-error: true appears on the gate-tooling checkout and on the rate-limit admission gate step. The fail-open design is deliberate and documented: a gate outage must not stop dispatch. Both failure paths emit ::error:: plus a step-summary warning and keep the real exit code. The gate is a cost/throttle control, not an authz or security control, so failing open does not open a security hole. It still trips the CI-weakening hard-stop, and a human must accept the fail-open design.
  • minor: The org PAT (GH_PAT_DON_PETRY, falling back to GH_PAT_WORKFLOWS) is exported as GH_TOKEN to scripts/agent-rate-limit-gate.sh and lib/agent-rate-limit.sh, which are checked out from combsmith/.github. Mitigations verified: (1) the ref cd0b16751454d2eb3486ec04477d7ee9cc96c425 exists and is an ancestor of main (main is 60 commits ahead), and the gate script is present at that ref; (2) actions/checkout@3d3c42e matches the v7.0.1 tag; (3) persist-credentials: false is set. A scan of the gate and its lib found no eval and no sending of GH_TOKEN to a non-GitHub host. The only external call (api.anthropic.com usage telemetry) uses CLAUDE_CODE_OAUTH_TOKEN, which this stub never provides, so that path fails open. Residual risk: any future SHA bump expands what the PAT is handed to and must be reviewed as a credential-exposure change. The gate can also comment on and label issues with the PAT (breaker escalation), which is expected.
  • minor: The autofix check FAILED. Prettier reformatted pr-review.yml (7 lines; the template's ' # NOSONAR' double-space comment style differs from the repo's prettier config), and the push was rejected with 'refusing to allow a GitHub App to create or update workflow ... without workflows permission'. The rejection is correct protective behaviour, but the synced template is not prettier-clean for this repo. Fix it in the source template in combsmith/.github (or exempt the synced stubs from prettier) rather than diverging the stub here. mergeStateStatus is BLOCKED with REVIEW_REQUIRED.
  • minor: The gate is invoked without --labels, so the exempt-label bypass (e.g. security) never applies to initiative-driver dispatches. Impact is limited: one deferred dispatch, which the daily schedule retries. This is a source-template issue in combsmith/.github, so fix it there.
  • minor: pr-review.yml moves its channel from pr-review/stable to pr-review/v1-stable, with agent_ref in lockstep; the tag exists in combsmith/.github-private. It also adds a job-level github.actor != 'dependabot[bot]' skip. If 'review / review' is a required status check, Dependabot PRs will report it as SKIPPED, which GitHub treats as passing. This is consistent with the org standard because Dependabot PRs have their own automerge path.
  • info: There are no expression-injection sinks. github.actor, github.repository and the issue number reach run: blocks only through env vars, and all are quoted. The dispatch step's target_repo moved from an inline ${{ github.repository }} to env TARGET_REPO, which removes a zizmor template-injection finding. Triggers are issues(closed, labeled), schedule and workflow_dispatch. There is no pull_request_target and no untrusted checkout, and permissions remain contents: read.
  • info: Both changed files are byte-identical to combsmith/.github main:standards/workflows/{initiative-driver,pr-review}.yml, so this is a genuine standards sync with no local divergence.

- **Stale CodeRabbit comment** (`IC_kwDORFz9wc8AAAABdcibUA`):
  - The review was skipped. Its only finding is a Merge Risk note that a security-labeled initiative could be deferred, because issue labels aren't passed to the rate-limit gate.
  - That concerns `.github/workflows/initiative-driver.yml`, a synced org-standard stub that is read-only on this `standards-sync` PR.
  - I posted a fresh `answered` disposition declining the change. The template in `combsmith/.github` is where it belongs.
- **`autofix` check failure:** the autofix push was rejected because the GitHub App token lacks `workflows` permission to update `pr-review.yml`. That is a token limitation, not a code defect, so I can't fix it from this PR. I noted it in the same comment.
- **Tests:** not run, since the diff is unchanged.
- **Files changed:** none.

Summary

Both stubs match the combsmith/.github source templates byte-for-byte. Every pin checks out: actions/checkout v7.0.1 is commit 3d3c42e, cd0b167 is an ancestor of .github main, and pr-review/stable and pr-review/v1-stable both point at e524d16. I found no secret exposure or injection, and the PR removes an inline ${{ github.repository }} from shell text. The PR is still escalated: the deterministic CI_WEAKENING hard-stop (two intentional fail-open continue-on-error steps) is a gate I will not override. On top of that, the gate counts every run, including job-skipped label events and issue closes that any outside user can trigger on this public repo, and it never gets --labels, so outsiders can exhaust the gate's budget and security-labelled epics can be deferred. All of these fixes belong in the source template and gate in combsmith/.github, not in this caller.

Downstream impact

This change is consumed by 5 downstream repo(s) that pin the affected reusable workflow / lib / prompt. Impacted consumers:

Impacted shared surfaces:

      .github/workflows/pr-review.yml
  - combsmith/markets (pins .github/workflows/pr-review.yml)
      .github/workflows/agent-ingress.yml

Findings

  • MAJOR: Deterministic hard-stop CI_WEAKENING_DETECTED: continue-on-error: true on the gate-tooling checkout and on the admission-gate step. This is intentional, documented fail-open behaviour: if the gate breaks, behaviour falls back to the ungated dispatch from before this PR, and the step emits ::error:: plus a step-summary warning. Nothing alerts on that signal, though. A silently broken gate leaves the job green, and the gate tooling is already 68 commits behind combsmith/.github main. Needs a human sign-off on the fail-open policy. (.github/workflows/initiative-driver.yml:115)
  • MAJOR: Outside users can exhaust the rate-limit gate's budget on this public repo. The gate's daily_run_budget (20/24h) and cooldown (10 min) count EVERY run of initiative-driver.yml (gh run list with no filter on event, conclusion or whether a dispatch happened). That includes issues: labeled runs for other labels, where the job is skipped, and issues: closed runs. Any GitHub user can open and close their own issue, so about 20 open/close cycles a day keep the gate deferring legitimate dispatches, including the 03:41 schedule safety net. Separately, applying initiative:auto together with other labels creates sibling runs within seconds. The cooldown counts those siblings, so arming an epic can itself be deferred. Fix in the gate or source template: count only runs that actually dispatched (for example, exclude skipped/cancelled conclusions or filter by a marker), and consider ignoring closed events from non-collaborators. (.github/workflows/initiative-driver.yml:158)
  • MAJOR: The gate is called without --labels, and the pinned gate (cd0b167, agent-rate-limit-gate.sh:444) applies its exempt-label bypass only when --labels is non-empty. An issues event on an epic carrying an exempt label (e.g. security) can therefore be deferred while a limit is active. The fix belongs in combsmith/.github standards/workflows/initiative-driver.yml, for example --labels "${{ join(github.event.issue.labels.*.name, ',') }}" passed via env. (.github/workflows/initiative-driver.yml:158)
  • MINOR: GH_PAT_DON_PETRY, a broad PAT that can dispatch workflows and label across repos, is exported as GH_TOKEN to bash code fetched from combsmith/.github. The trust boundary is acceptable: the code is the org's own, pinned to an immutable SHA that I verified is on main, and checked out with persist-credentials: false. Every future bump of the pinned ref still effectively grants that code the PAT, so bumps need security review. A narrower read-only token would be better for the gate's gh run list / issue reads. (.github/workflows/initiative-driver.yml:143)
  • MINOR: ARL_TRACKING_ISSUE resolves to github.event.issue.number ahead of vars.INITIATIVE_DRIVER_TRACKING_ISSUE. On issues: closed, a tripped breaker posts its marker comment and applies needs-human-review (using the PAT) to whatever issue was just closed, which may be an issue an outsider opened. The escalation lands in the wrong place. Prefer the configured tracking-issue variable first. This is a source-template concern. (.github/workflows/initiative-driver.yml:146)
  • INFO: Pins verified via the GitHub API: actions/checkout@3d3c42e is tag v7.0.1, and combsmith/.github@cd0b167 exists and is an ancestor of main. The inline comment main @ 2026-10-02 does not match the commit date (2026-09-30T00:09:55Z), which is cosmetic. (.github/workflows/initiative-driver.yml:117)
  • INFO: The dispatch step now passes github.repository via env (TARGET_REPO) instead of inline expression expansion in shell text, which removes a zizmor template-injection finding. github.actor and the issue number are also passed via env. (.github/workflows/initiative-driver.yml:176)
  • INFO: The pr-review.yml channel bump to @pr-review/v1-stable (agent_ref moved in lockstep) is a no-op today: both tags resolve to e524d16. The new job-level Dependabot skip guard means Dependabot PRs get no AI review. review / review is not a required check in the main ruleset, so the skip bypasses no merge gate. secrets: inherit to the first-party reusable is unchanged from before. (.github/workflows/pr-review.yml:58)
  • INFO: The autofix check failed because prettier wanted to reformat pr-review.yml and the App token's push was rejected for lacking the workflows permission. autofix is not a required check, and all required checks pass (SonarCloud, CodeQL, AgentShield, dependency-audit, build-and-test, coverage). The pr-review agent also reported its advisory bots were rate-limited at this SHA, which withholds auto-approval until 2026-10-21T03:09Z.
  • INFO: Since the prior reviewed SHA bb6aa8d, the only change is a merge of main (commit 580ab53, already reviewed on main). The PR's own workflow diff is unchanged.

Re-read the current CodeRabbit summary. The only finding is the Merge Risk note: a security-labeled initiative can be deferred despite its exemption, so issue labels should be passed to the gate.

Declined, with no code change. .github/workflows/initiative-driver.yml is an org-standard stub with the header SOURCE OF TRUTH: combsmith/.github/standards/workflows/initiative-driver.yml. It is synced verbatim and is read-only in this standards-sync PR. Editing it here would drift from the template, and the next sync would revert the edit. The fix belongs in the template at https://github.com/combsmith/.github/blob/main/standards/workflows/initiative-driver.yml. The other sections of the comment are the rate-limit notice, the walkthrough and passing pre-merge checks, none of which need action.

The two continue-on-error steps do not weaken CI: they sit on a NEW operational rate-limit gate in a non-PR dispatch workflow, which agent-rate-limits.md §3.1 sets as fail-loud/fail-open, and no existing check or threshold is removed. The PAT goes to code pinned at commit cd0b167 of combsmith/.github, which is an ancestor of its main branch, with persist-credentials false. All action, tag and SHA pins resolve, and every untrusted context value goes through env. The remaining issues are minor logic defects that belong in the upstream template.

Findings

  • minor: Accepted at Tier 3: continue-on-error on the gate-tooling checkout and the gate step is the documented fail-open design (combsmith/.github standards/agent-rate-limits.md §3.1, lines ~128-130, ~177-180). It adds a throttle where there was none and removes or relaxes no PR check, test or threshold. The only cost is that a persistently broken gate shows up as ::error:: annotations and step-summary entries, never as a red job. An upstream alert (for example, open an issue after N consecutive ungated runs) is recommended.
  • minor: The write-capable org PAT (GH_PAT_DON_PETRY || GH_PAT_WORKFLOWS) is exported as GH_TOKEN to bash code from combsmith/.github. The exposure is contained: the ref is the immutable commit cd0b16751454d2eb3486ec04477d7ee9cc96c425, which is an ancestor of combsmith/.github main (compare status 'ahead'), so it is reviewed and merged code. persist-credentials is false. The script makes no curl/wget calls and contacts no external hosts. This is the same trust domain as the org reusable workflows already called with secrets: inherit. Any SHA bump must still arrive through a reviewed standards-sync PR.
  • minor: Confirmed: the stub never passes --labels, and the gate checks exempt labels only when --labels is non-empty (gate.sh ~L444). Issues labelled with an exempt label can therefore be deferred while a limit is active. Fix this upstream in standards/workflows/initiative-driver.yml.
  • minor: ARL_TRACKING_ISSUE prefers github.event.issue.number. The repo is public and an issue author can close their own issue, so anyone can trigger the run. While the breaker is open, argate_escalate posts the breaker marker and applies needs-human-review on that arbitrary issue, not on a stable tracking issue. The impact is low: a marker comment and a label, written with the PAT. Fix this upstream by giving vars.INITIATIVE_DRIVER_TRACKING_ISSUE precedence.
  • info: Verified: actions/checkout@3d3c42e is the v7.0.1 tag. pr-review/v1-stable exists in combsmith/.github-private (e524d16). Untrusted values (github.actor, github.repository, issue number) reach the shell only through env. TARGET_REPO now goes through env, which removes the earlier template-injection pattern. There is no pull_request_target, and permissions stay contents: read.
  • info: pr-review.yml is a verbatim stub. It re-pins the channel and agent_ref in lockstep to pr-review/v1-stable and adds the documented Dependabot skip guard (a skipped job counts as passing for required checks). The workflow name, job id, permissions and triggers are unchanged.
