Problem

Compliance: verbatim-stub-drift-initiative-driver.yml

From the issue: Category: ci-workflows Severity: error Check: verbatim-stub-drift-initiative-driver.yml

Risk

Medium — changes GitHub Actions workflow behavior, which is exercised only post-merge; verify via the affected workflow runs.

Test plan

No test files were added or updated. Verification: bash scripts/dev-lead-lint.sh (shellcheck --severity=warning) ran pre-commit; the existing CI (bats + lint) guards the change.

Rollback

Revert this PR. No non-revertible side effects (no tags, migrations, or external state).

Monitoring

Watch the affected workflow run(s) in the Actions tab and this PR's Lint check for regressions.

Closes #548

View guided diff Turn on auto-fix

Summary by CodeRabbit

  • Automation
    • Initiative-driver runs now check agent rate limits before dispatching. Runs explicitly marked for deferral are skipped; if the rate-limit check fails or its tooling is unavailable, dispatch proceeds.
    • Workflow dispatch continues to target the designated repository, with the target passed through the workflow environment.

Addressed 0 threads. There were no open review threads, no `CHANGES_REQUESTED` reviews, and no failing CI checks.
- **CI:** The one cancelled `dev-lead / dispatch` run is a superseded run, and a newer one is still in progress. I did not treat it as a blocker.
- **CodeRabbit summary comment:** I replied `invalid`. It said the workflow differs from the template by one blank line. I fetched `standards/workflows/initiative-driver.yml` from `combsmith/.github` and ran `diff` against `.github/workflows/initiative-driver.yml`. The two files are identical, so no change was needed.
- **Codex comment:** I replied `informational`, because Codex completed its review with no findings.
- **SonarCloud comment:** I replied `informational`, because the quality gate passed.
- **Own comments:** I left the existing `don-petry` dev-lead comments alone.
I made no code changes and no commits, so I did not run the test suite. The diff is a single synced workflow stub, and I had no reason to change it.
Files changed: none.

Cascade: triage → audit (triage: haiku 4.5 [sonnet 5.5, sonnet 5] → deep: opus 5.5 [opus 4.8, sonnet 5.5] + duck: gemini-3.8-flash [sonnet 5.5] → audit: opus 5.5 [opus 4.8, opus 4.7])

Summary

The PR syncs .github/workflows/initiative-driver.yml to the canonical template, and the file at head b97bf35 is byte-identical to combsmith/.github standards/workflows/initiative-driver.yml (checked by diff). I found no injection, secret-leak or supply-chain defect: both pinned SHAs are real, the combsmith/.github ref is an ancestor of main, persist-credentials is false, and every untrusted expression goes through env. The two added continue-on-error: true steps still trip the deterministic CI_WEAKENING_DETECTED hard stop, which this tier cannot override, so a human must approve the fail-open design.

Findings

  • major: HARD STOP: two added steps set continue-on-error: true, the gate-tooling checkout and the agent rate-limit gate. Neither is a test, lint or scanner gate. Both are the template's documented fail-open admission control, so this is not a real weakening of CI quality gates. The deterministic hard stop still requires human sign-off. Side effect: a persistently broken gate leaves the job green, and the only signals are the ::error:: annotation and the step summary. (.github/workflows/initiative-driver.yml:115)
  • minor: The org PAT (GH_PAT_DON_PETRY || GH_PAT_WORKFLOWS) is now used to check out combsmith/.github and is passed as GH_TOKEN to scripts/agent-rate-limit-gate.sh from that repo, which sources lib/agent-rate-limit.sh. Mitigations I verified: the ref is pinned to the full SHA cd0b16751454d2eb3486ec04477d7ee9cc96c425, which exists and is an ancestor of combsmith/.github main (main is 60 commits ahead). persist-credentials is false. actions/checkout is pinned to 3d3c42e5aac5ba805825da76410c181273ba90b1, which matches the v7.0.1 tag. Residual risk: the PAT has broad write scope for what is a read-mostly gate, and every pin bump needs security review. (.github/workflows/initiative-driver.yml:122)
  • minor: The issues:closed trigger already existed, but any issue author can fire it, including non-collaborators closing their own issues. The gate derives cooldown and daily-budget counters from this workflow's run history, so an outsider who repeatedly opens and closes issues can use up the budget and make legitimate dispatches defer. That throttles the fleet; it does not escalate privilege. ARL_TRACKING_ISSUE also defaults to the event's issue number, so if the breaker trips, the PAT posts the breaker comment and needs-human-review label on an issue that could belong to an outsider. Impact is low, and the daily schedule acts as a safety net. (.github/workflows/initiative-driver.yml:143)
  • minor: The dispatch guard steps.arl_gate.outputs.decision != 'defer' fails open on an empty or unexpected decision. The gate script's header says it always exits 0 and carries its decision in GITHUB_OUTPUT, so the error branch is defensive. Fail-open is intended per standards/agent-rate-limits.md. (.github/workflows/initiative-driver.yml:167)
  • info: Expression-injection hygiene is good. github.actor, github.repository and the issue number reach shell only through env vars and are double-quoted. The old inline ${{ github.repository }} in the dispatch run block now goes through TARGET_REPO. The workflow has no pull_request_target trigger, and permissions stay at contents: read. (.github/workflows/initiative-driver.yml:173)
  • info: The file is byte-identical to the org template, which resolves compliance issue Compliance: verbatim-stub-drift-initiative-driver.yml #548. Required checks (CodeQL actions/python, gitleaks, workflow regression guards, SonarCloud, AgentShield) passed. Nothing has exercised the new gate path before merge. (.github/workflows/initiative-driver.yml)

Dev-Lead — fix-reviews (no-changes)

Agent reasoning
Addressed 0 threads. There were no open review threads, no failing CI checks and no `CHANGES_REQUESTED` reviews. I posted fresh dispositions on the two stale issue comments.
- **`IC_kwDORsnIRM8AAAABdLnJlQ` (Codex summary):** the review on `8c367a6` shows "Failed" with no finding text. I marked it `informational`.
- **`IC_kwDORsnIRM8AAAABdLnUJg` (CodeRabbit summary):** I marked it `invalid`.
  - Its Merge Risk section says the workflow differs from the central template by one blank line. I fetched `standards/workflows/initiative-driver.yml` from `combsmith/.github` and diffed it against `.github/workflows/initiative-driver.yml`. The two are identical once CRs are stripped, so there is no drift.
  - The same diff answers the inconclusive Linked Issues check, which asked for a byte-for-byte comparison.
  - The review-limit notice has nothing to act on.
Test verification: I didn't run the suite, because I made no code changes.
Files changed: none. No commits were made.

Summary

Head 8c367a6 is only a merge of main into b97bf35. The PR's one changed file (.github/workflows/initiative-driver.yml) is unchanged since the prior audit and is still byte-identical to combsmith/.github standards/workflows/initiative-driver.yml (checked again by diff). I found no injection, secret-leak or supply-chain defect: both pins are valid, persist-credentials is false, untrusted expressions go through env, permissions stay contents: read, and there is no pull_request_target trigger. The two continue-on-error: true steps are the template's documented fail-open admission gate, not a weakened test, lint or scanner gate. They still trip the deterministic CI_WEAKENING_DETECTED hard stop, so a human must sign off.

Findings

  • MAJOR: HARD STOP: continue-on-error: true is set on the 'Checkout agent-rate-limit gate tooling' step (line ~115) and on the 'Agent rate-limit admission gate' step (line ~133). Neither one is a test, lint or scanner gate. Both are the org template's documented fail-open admission control (standards/agent-rate-limits.md), so CI quality gates are not really weakened. The deterministic hard stop still needs human approval. A persistently broken gate leaves the job green; the only signals are the ::error:: annotation and the step summary.
  • MINOR: The PAT (GH_PAT_DON_PETRY || GH_PAT_WORKFLOWS) checks out combsmith/.github and is exported as GH_TOKEN to scripts/agent-rate-limit-gate.sh, which sources scripts/lib/agent-rate-limit.sh. Verified: the ref cd0b16751454d2eb3486ec04477d7ee9cc96c425 is a full SHA and an ancestor of combsmith/.github main (68 commits behind main, not diverged). actions/checkout 3d3c42e5aac5ba805825da76410c181273ba90b1 matches tag v7.0.1. persist-credentials is false. At that SHA the token is used only for gh run list, gh issue view/comment/edit. The token-budget telemetry fetch runs only when AGENT_TOKEN_BUDGET_TELEMETRY_CMD/_FILE is set, and this workflow sets neither, so no external endpoint receives the PAT. Residual risk: the PAT is broadly scoped for a read-mostly gate, and every bump of the pinned SHA needs a security review.
  • MINOR: Any user who can open and close issues can fire the issues:closed trigger. The gate derives its cooldown and daily-budget counters from this workflow's run history, so repeated close events from outsiders can use up the budget and defer legitimate dispatches. The daily schedule is a safety net. ARL_TRACKING_ISSUE defaults to the triggering issue number, so if the breaker trips, the PAT may post the breaker comment and the needs-human-review label on an issue opened by an outsider. The comment text is fixed and contains no user-controlled content. This throttles the fleet but does not escalate privilege.
  • MINOR: The dispatch guard steps.arl_gate.outputs.decision != 'defer' fails open on an empty or unexpected decision. This is intended: the gate script header (line 33) states that it ALWAYS exits 0 and carries its decision in GITHUB_OUTPUT, so the || { rc=$?; ... } error branch is only defensive.
  • INFO: github.actor, github.repository and the issue number reach shell only through env vars and are double-quoted. The old inline ${{ github.repository }} in the dispatch run block is replaced by TARGET_REPO, which closes a zizmor template-injection finding without changing the dispatch target.
  • INFO: mergeStateStatus is BLOCKED (REVIEW_REQUIRED). The Codex review on 8c367a6 failed and CodeRabbit was rate-limited. All reported checks pass: CodeQL, gitleaks, SonarCloud, AgentShield and Workflow regression guards. The merge commit pulled in ci.yml and test-script changes that already exist on main; they are not part of this PR's diff.

Re-read the current CodeRabbit body. Its findings:

  1. Merge Risk (Moderate): says the workflow differs from the central template by one blank line. This is invalid for the current head: I fetched standards/workflows/initiative-driver.yml from combsmith/.github and ran diff against .github/workflows/initiative-driver.yml at 7b72956. They are byte-identical. The note was computed up to the older b97bf commit.
  2. Linked Issues check (inconclusive): asks for evidence of byte-for-byte equality with the canonical template. The same diff shows zero differences, so the check is satisfied. The template lives in combsmith/.github, not this repo, which is why the reviewer could not see it.
  3. The "Review limit reached" block is a rate-limit notice only; the other pre-merge checks passed.

No code change needed; the stub is a verbatim synced copy and stays read-only.

P1 Badge Count actual dispatches instead of caller runs

Passing this workflow as the history source makes the pinned gate count every invocation— including cancelled runs and successful runs where the dispatch step was skipped—as a prior dispatch for both cooldown and the 20-per-day budget. For example, a burst of 20 issue events can exhaust the daily budget even if only one central dispatch occurred, after which later dependency-unblocking events are deferred for up to 24 hours. The history used for admission needs to distinguish runs that reached the central dispatch from gate-only attempts.

Declined — change the template instead: this stub is synced verbatim from https://github.com/combsmith/.github/blob/main/standards/workflows/initiative-driver.yml, so a change here drifts from the template and the next sync reverts it. (The gate-side points, e.g. history source, failure-streak handling and escalation dedup, belong in the pinned gate script / template in combsmith/.github.)

# a job failure (#640, AC #1/#2). Fail-OPEN: an empty/missing decision (the
# gate step or its tooling checkout failed) still dispatches, because an
# outage of the gate must never stop the fleet (ADR §7 fail-safe direction).

P1 Badge Preserve the failure streak when a dispatch is deferred

When the circuit breaker opens after consecutive failures, this condition skips the dispatch but leaves the workflow run successful. The pinned gate computes its next failure streak from this workflow's conclusions and stops at the most recent success, so that successful no-op immediately clears the breaker; the following attempt can dispatch after the shorter cooldown instead of respecting the configured 30-minute breaker backoff. Deferred runs need a conclusion or history marker that does not reset the dispatch failure streak.

P1 Badge Forward triggering issue labels to the admission gate

For a close event on a security-labeled issue, or when initiative:auto is added to an issue that already has the security label, this invocation passes the actor but not the event issue's labels. The pinned gate therefore cannot apply its configured security-label exemption and may defer a run that its policy says must never be blocked. Pass the triggering issue labels through the gate's --labels option.

P2 Badge Use the configured tracking issue for issue-triggered runs

On every issues trigger, the event issue number wins over INITIATIVE_DRIVER_TRACKING_ISSUE, so a breaker opened while processing a close event posts its escalation comment and needs-human-review label on the just-closed issue rather than the configured operations issue. Different close events also select different issues, preventing a single durable breaker record and scattering human-clearance markers across unrelated or closed work items. Prefer the configured tracking issue when present instead of restricting it to scheduled/manual runs.

P2 Badge Prevent duplicate circuit-breaker escalation comments

Enabling --tracking-issue activates the pinned gate's per-agent escalation path, but that implementation checks only the issue body for its deduplication marker and then writes the marker into a comment. Consequently the marker is never found on a later breaker opening, so each opening posts another identical comment even if the prior escalation has not been cleared. The pinned gate should inspect existing comments, as its token-budget escalation path already does, before this caller enables issue escalation.

Acknowledged: this is the Codex review status summary (code review completed on 7b72956). It carries no findings of its own; the five findings it refers to are in review threads, which were declined because initiative-driver.yml is a verbatim synced org-standard stub (change belongs in the template in combsmith/.github). No action needed here.

Automated review — escalated to human

The automated review cascade escalated this PR to a human reviewer at review cycle 2/3 (risk: HIGH, reviewed commit 7b72956fc233116af0cf1e7ec32a64452686a196).

Why: the cascade could neither approve the PR nor auto-request fixes, so it requested human review via CODEOWNERS and set the needs-human-review label. A human should review the PR, or remove the needs-human-review label to re-engage the automated cascade.

Reviewer summary: I checked .github/workflows/initiative-driver.yml at 7b72956 and it is byte-identical to combsmith/.github standards/workflows/initiative-driver.yml, so the PR does resolve #548. The paranoid pass found no injection, no new external trigger surface, and no pull_request_target; it actually removes an inline ${{ github.repository }} template expansion. All third-party code is pinned to verified SHAs. I am still escalating to a human for two reasons: the deterministic CI_WEAKENING_DETECTED hard-stop (two continue-on-error: true lines) is a gate that blocks bot approval, and the PR adds a run step that hands the write-capable dispatch PAT to bash code checked out from another repo. That code is first-party and SHA-pinned, but a human should explicitly accept it.

This note is updated in place on re-escalation; it is not re-posted.

dev-lead is withholding action on this item.

It is labeled needs-human-review (flagged for human review — this label is applied by automation as well as by people, so an item can become held without anyone noticing), so dev-lead will not pick it up while that label is present. This notice is posted once so the withhold is visible rather than looking like a stalled run.

To re-enable automated pickup: remove the needs-human-review label.
