fix(sdk): a health check registered after startup runs in /readyz, and registerHealthProbe returns a remover - #1135
scope.registerHealthProbe(probe) lets a plugin add a check to /readyz. @frontmcp/plugin-skilled-openapi uses it to report its bundle. Review found that it only worked while the server started: a check registered after the scope was ready never ran.
Confirmed first, with tests that fail on the base branch:
a check registered after createForGraph() resolved was missing from getReadyz(), which answered ready though the check failed;
over real HTTP (the new e2e), a check a tool registered after the server started was missing from /readyz, which answered 200.
Why: the health service copied the scope's checks once, when the scope finished starting (autoDiscoverProbes). A splitByApp server's combined service copied each app's checks again, when the server wired its health routes. Nothing read the list after that. #1091 added registerHealthProbe that way on purpose, and its doc said so ("the health service takes the checks registered while the server starts"). It was the simplest wiring for the plugin, which registers from a provider factory during startup. Nothing needed a later check then.
What changes:
Every /readyz reads the checks again. The health service no longer copies the scope's checks. It keeps the scope's list itself and reads it at the start of each check. A splitByApp server's combined service reads each app's service the same way. So a check registered at any time runs from the next /readyz on. Examples: a feature started on demand, an app loaded on its first call, a plugin that starts something after the server is up.
A running check isn't affected. Each /readyz takes its list of checks when it starts. A check registered or removed while it runs (even from inside one of its checks) shows up in, or drops out of, the next one.
registerHealthProbe returns a function that removes the check. This follows scope.onDispose(), which already returns a remover. The return type goes from void to () => void, so existing callers still compile and work.
Each call removes its own registration, once. A check registered twice keeps the other registration, and calling the remover again does nothing.
A name registered twice: the startup rule is kept. At startup the SDK neither refused nor replaced a duplicate name. Both checks ran and both counted toward the status, and probes showed the later one's result. A check registered after startup gets the same rule. The JSDoc and the catalog now say to give each check its own name.
Both HTTP paths. The Node server's /readyz route and the fetch handler's (Workers, Vercel Edge, Deno, which @frontmcp/edge uses) both call the scope's health service. @frontmcp/edge has no health service of its own, so it needed no change. /healthz runs no checks and is unchanged.
Scenarios: a check registered after startup, by a tool over HTTP and directly on the scope; its remover; a name registered at startup and again later; a check registered and removed during a running check; a splitByApp server; the fetch handler's /readyz.
User-facing change
What changed:
scope.registerHealthProbe(probe) works at any time. A check registered after the server started runs on the next /readyz.
It returns a function that removes the check: () => void, where it returned void.
Before → after:
A check registered after startup: never run, so /readyz answered 200 ready while it failed → run on the next /readyz, which answers 503 not_ready while it is unhealthy.
The same on a splitByApp server, and through the fetch handler (Workers, @frontmcp/edge).
const remove = scope.registerHealthProbe(probe): undefined → a function; remove() takes the check off the next /readyz.
frontmcp.dev pages affected (if known): the health and readiness page (checks plugins register), and the plugin authoring page's server lifecycle hooks (registerHealthProbe).
Skills catalog entries updated:
frontmcp-production-readiness/references/health-readiness-endpoints.md: checks registered at any time, the remover, the 1.11.0 behavior.
frontmcp-development/references/create-plugin-hooks.md: the new signature, when to register, the remover, duplicate names.
Follow-ups, left out of this PR:
Duplicate names: two checks with one name both run and both count toward the status, but probes shows only the later one. A failing earlier check can make /readyz answer 503 with no failing entry in probes. Refusing the second name, or replacing the first, is a separate decision; it changes startup too. The same goes for a health.probes entry named like a plugin's check.
/healthz and /readyz are plain routes, not hookable flows.
Tests
Added:
libs/sdkhealth.scope-probes.spec.ts (+5, 6 in all), on a real scope:
a check registered after the scope is ready runs on the next /readyz, which answers not_ready;
the remover takes off its own registration, once, and leaves a second registration of the same check;
a name registered at startup and again after it: both run, probes shows the later, and removing it shows the startup one again;
a check that registers one check and removes another while /readyz runs: that /readyz keeps its checks, the next one has the change;
a splitByApp server's Node /readyz (real HTTP) runs a check an app's scope registered after startup, then drops it once removed.
libs/sdkhealth.service.spec.ts (+2): the scope's checks are read at every check; a combined service reads another service's checks at every check (registerProbeSource). One test no longer reaches into a private field.
libs/sdkweb-fetch-handler.http-hardening.spec.ts (+1): the fetch handler's /readyz runs a check registered after the handler was built, and answers 503.
e2e demo-e2e-standalonehealth-probe-after-startup.e2e.spec.ts (2), over real HTTP, with a new main-health-probes.ts and a late-probe app whose tools start and stop a feature:
a check the feature registers after startup is on the next /readyz, which answers 503 while it fails;
the function registerHealthProbe returned takes it off /readyz again.
libs/edge jest (no changes there): 10 suites, 109 tests pass.
e2e demo-e2e-bundle-size: 12 suites, 63 tests pass, with the budgets set to the measured sizes:
create() + one tool: 1,295,269 → 1,295,400 (+131);
create() + WebMcpPlugin + one tool: 1,302,499 → 1,302,630 (+131);
Vite initial chunk: unchanged at 768,075.
The growth is the remover and the probe list read at each check.
nx run <project>:typecheck passes for sdk, plugin-skilled-openapi and demo-e2e-standalone. tsc --noEmit -p libs/sdk/tsconfig.lib.json passes.
Prettier and ESLint on the changed files: clean, with no new warnings.

New Features
Health checks can now be registered or removed while the server is running. Changes appear in subsequent /readyz responses, while checks already in progress continue with their original set.
Registering a health check now returns a function to remove that specific check. Duplicate names still run and count toward readiness, though only the later result is displayed.
Documentation
Updated health-check guidance to describe when checks take effect, how to remove them, and how duplicate names are reported.
…HealthProbe returns a function that removes it
scope.registerHealthProbe() only worked while the server started: the health
service copied the scope's checks once, when the scope was ready, and a
splitByApp server's combined service copied each app's checks again when it
wired its routes. A check a feature, a lazily loaded app or a plugin
registered later never ran, so /readyz answered ready while it failed.
Every /readyz now reads the scope's checks again when it starts, on the Node
route and in the fetch handler (Workers, @frontmcp/edge), and a splitByApp
server's combined service reads each app's service the same way. A running
check keeps the checks it started with. registerHealthProbe returns a
function that removes its own registration, as onDispose() does. A name
registered twice keeps the startup rule: both run, and /readyz shows the
later one's result.
The e2e drives it over HTTP: a tool starts a feature that adds a check after
startup, and stops it. The browser bundle grows 131 bytes; its budgets are
set to the measured sizes.
The SDK now reads health probes dynamically for readiness checks and returns a callback to remove each scope registration. Tests and documentation cover post-startup registration. The PR also adds a standalone E2E scenario and raises two browser bundle size limits.

…anged, not one per entry (#1136)
* fix(sdk,plugins): a bundle swap sends one list_changed per list it changed, not one per entry
Applying a Skilled OpenAPI bundle registers and removes skills, operation tools and the skills'
skill:// resources one by one, and each registry change sent every connected client a
list_changed: about 25 for a modest bundle, and again on every poll that pulled the bundle
being served.
The scope gets batchListChanges(change): while the change runs, the list_changed its registry
changes would send are held, and once it ends (also when it throws) each list it changed is
announced once. A batch inside another joins it. The batch follows the async call chain, so a
change made by other code meanwhile is sent at once, and a change outside a batch is sent as
before. The notification service, the 2026-07-28 subscription streams and the live surface's
tool list version all hold their announcements for it.
The plugin applies each bundle in one batch. A pull of the bundle being served (the same
content) leaves the registries alone and sends nothing, a rollback restores the operation tools
only when it replaced them, and the first bundle that loads sends skills/list_changed even
with no skills, since the skill surfaces stop answering that no bundle is loaded.
* fix(sdk,plugins): a rolled-back bundle sends no list_changed, and a swap sends no tools/list_changed for internal tools
A bundle that failed part-way and was rolled back to the served one still sent a skills and
resources list_changed, though the lists were back as they were. The batch callback now gets a
ListChangeBatch: listsRestored() drops what the batch changed so far, and a nested batch hands its
changes to the outer one, so restoring an inner batch drops only its own. The plugin calls it when
the rollback restored every skill; a rollback that can't restore one still announces the lists it
left changed.
Every swap also sent tools/list_changed for the operation tools, which are internal and never in
tools/list. Registering or removing an internal tool through registerToolInstance() /
unregisterToolInstance() now leaves the lists unchanged: no tools/list_changed, and the live
surface's tool list version stays.
The bundle-size budgets are set to the measured sizes, which include the batch added before.
* test(e2e): the browser bundle budgets are the sizes measured on top of #1135
The batch of list changes and the internal-tool check add 864 bytes to create() + one tool, 865 with WebMcpPlugin, and 516 to the Vite initial chunk. The scope's batchListChanges doc says a nested batch hands its changes to the outer one.
