fix(auth, adapters, plugins, edge): operators can tell why a pull or push token was refused, while callers still get the generic answer - #1130
The saas source checks tokens against the issuer's JWKS: the pull token before every pull, and the token of each push. Until now, every failed check ended in one generic reason: not a valid token from issuer "…" signed by a key in … (no_provider_verified). An expired token, a token from another issuer, one signed by a key the JWKS doesn't publish, a bad signature: all read the same. What the verifier knew went only to a debug log. Operators couldn't tell from the logs or /readyz which check failed.
Each failed check now has a name, tokenFailure, from a closed set:
tokenFailure
The token
expired
its exp has passed
not_yet_valid
its nbf is in the future
wrong_issuer
its iss is not expectedIssuer, or it has none
wrong_audience
its aud, or its resource, doesn't name expectedAudience
unknown_key
its kid is not in the JWKS
bad_signature
the key under its kid doesn't verify it
unsupported_algorithm
it isn't signed with an asymmetric algorithm (HS256, for example)
missing_claim
it has no exp; a push token also needs iat and jti
malformed
it isn't a JWT, or a claim has the wrong type
jwks_unavailable
not checked: the JWKS couldn't be read, or has no usable signing key
How the name is found.
@frontmcp/auth: JwksService.verifyTransparentToken sets a new failure on a refusal, from the error jose throws (its code, and the claim for a claim check). The weak-RSA fallback sets it too. With several providers, it names the check of the provider that got furthest: a claim check means that provider's key verified the signature. The error text is unchanged.
The saas source adds wrong_audience, for its own aud and resource checks, and jwks_unavailable. A JWKS it can't read now throws SaasJwksUnavailableError, with the same message as before. SaasPullTokenRejectedError and SaasPushRefusedError carry tokenFailure, and tokenFailureOf(error) reads it from any of the three.
Where operators see it.
A pull logs [saas-source] saas:<endpoint>: pull token check failed (expired) at warn level, on a start, a poll and a refresh. The line never has the token.
The sync outcome: a failed outcome has tokenFailure. It reaches the plugin through the errors a start or a refresh throws, the source's pull failure listeners (a new second argument), and the origin of a cached bundle served at a start (BundleOrigin.tokenFailure).
/readyz: the skilled-openapi check shows it in details.lastSync.tokenFailure. The check is degraded while an earlier bundle serves. An unhealthy check (no bundle loaded) now carries details.lastSync too, so a server that starts with an expired pull token says expired.
Redaction: tokenFailure is a closed code, so it has nothing to redact. The reason beside it still goes through publicLoadFailureReason, as before.
A push logs [saas-source] push refused (wrong_issuer): … (warn), or bundle push not checked (jwks_unavailable): … (error).
On a Worker, a failed Cron run's error says it: [frontmcp/edge] Cron refresh: the bundle pull failed (token check: expired): ….
What callers see doesn't change.
A refused push token answers 401 invalid_token, the push token was refused, whatever check refused it.
The tools' SKILL_BUNDLE_NOT_LOADED reason, and the check's error, keep their text: neither names the check. The pull token error's message is unchanged for that reason.
A push while the JWKS can't be read already answers 503 temporarily_unavailable, and keeps doing so. I checked: the push flow answers 503 for any error that isn't a token refusal, and an unreadable JWKS is one. 503 is right:
An unreadable JWKS is the server's problem, not the token's. A 401 would tell the SaaS its token is bad. A SaaS that drops a push on 401 would lose the bundle, while 503 says to retry.
Only the log changes: it now names jwks_unavailable.
Scenarios: B2 (the saas source's token checks) and B4 (a failure visible beyond the logs, in /readyz and the Cron run).
User-facing change
What changed:
The log names the check a pull or push token failed, and /readyz names the check a pull token failed: lastSync.tokenFailure.
The skilled-openapi check, when unhealthy, carries details.lastSync.
A managed Worker's failed Cron run names the token check in its error.
@frontmcp/auth: VerifyResult.failure and the TokenVerifyFailure type.
@frontmcp/adapters/skills: BundleTokenFailure, tokenFailureOf, SaasJwksUnavailableError, and SaasPullTokenRejectedError (exported now). SaasPullTokenRejectedError and SaasPushRefusedError have tokenFailure. BundleOrigin.tokenFailure, and a second argument to BundlePullFailureListener.
@frontmcp/plugin-skilled-openapi: BundleSyncOutcome's failed variant has an optional tokenFailure.
Before → after:
A refused pull token in the log: only the generic reason → also [saas-source] saas:<endpoint>: pull token check failed (<tokenFailure>) at warn level.
A refused push token in the log: [saas-source] push refused: … → [saas-source] push refused (<tokenFailure>): …. A push while the JWKS can't be read: bundle push not checked: … → bundle push not checked (jwks_unavailable): ….
/readyz while a pull failed on its token: lastSync: { status: 'failed', reason, cacheFallback, at } → the same, plus tokenFailure. An unhealthy check: details: { state: 'failed' } → details: { state: 'failed', lastSync }.
A Worker's Cron error: the bundle pull failed: … → the bundle pull failed (token check: expired): … when a token check failed.
The push answers, the tools' answers, and the check's error: unchanged.
frontmcp.dev pages affected (if known):/reference/plugins/skilled-openapi (troubleshooting, the health check, pushed bundles), /reference/deployment/edge (Cron runs, pushed bundles), the health and readiness page.
The plugin's README has a new troubleshooting section, "When a pull or push token is refused", and the edge's README is updated too.
Tests
Added:
@frontmcp/auth, jwks.service.failure.spec.ts: each failure, from tokens signed with test keys: expired, not_yet_valid, wrong_issuer (another iss, and none), missing_claim, unknown_key, bad_signature, malformed (not a JWT, and an exp that is a string), unsupported_algorithm (HS256), jwks_unavailable. Also: with two providers, in either order, the one that got furthest names it; an accepted token has none; and the weak-RSA fallback names expired, not_yet_valid, wrong_issuer, missing_claim and bad_signature. jwks.service.missing-exp.spec.ts now expects failure too.
@frontmcp/adapters:
saas-pull.source.verification.spec.ts: a refused pull token carries each reason, wrong_audience for aud and resource included. The message clients read doesn't name it. The warn line has the source id and no token. An unreachable JWKS is jwks_unavailable, and so is one that answers 503, isn't JSON, or has no keys or no usable key.
saas-pull.source.spec.ts: the cached bundle served because the JWKS was down carries tokenFailure: 'jwks_unavailable'. The pull failure listeners get the token check a poll failed on (unknown_key after a key rotation), and none for a 502.
saas-pull.source.push.spec.ts: each push refusal carries its reason (wrong_audience, wrong_issuer, expired, missing_claim, bad_signature, unknown_key), and the replay guard's refusals carry none. The message names it, an unreadable JWKS throws SaasJwksUnavailableError, and tokenFailureOf reads each.
Plugin:
bundle-sync.service.spec.ts: the outcome names the token check on a start that loads no bundle, a refresh, a poll, a start on the cached bundle (jwks_unavailable), and a refresh whose JWKS can't be read. A pull that fails for another reason has none.
bundle-load-state.spec.ts: a stale check carries lastSync.tokenFailure, and an unhealthy one carries lastSync.
bundle-push.flow.spec.ts: a token refused as expired, wrong_issuer, wrong_audience, unknown_key, bad_signature or malformed answers the same 401, and the answer doesn't name the check while the log does. An unreadable JWKS answers 503 and logs jwks_unavailable.
bundle-push.integration.spec.ts (a real server): four tokens refused by different checks get the same 401 body.
last-good-bundle.integration.spec.ts (a real server, read through the SDK's health service): a pull token from another issuer makes the check unhealthy with lastSync.tokenFailure: 'wrong_issuer', and error doesn't name it. A refresh whose JWKS can't be read is failed with jwks_unavailable, and the check is degraded.
Edge, index-wiring.spec.ts: a Cron run whose pull token expired, or whose JWKS couldn't be read while the cache serves, throws with (token check: …).
demo-e2e-skilled-openapi, saas on Node:
A server whose pull token is for another audience, from another issuer, expired, signed by another key, or signed under an unknown kid shows wrong_audience, wrong_issuer, expired, bad_signature or unknown_key in /readyz's lastSync. The log has the warn line and not the token. The tools' reason doesn't name the check.
A start during an outage shows jwks_unavailable while the cache serves.
A push with a token from another issuer gets the generic 401 invalid_token, and the log says push refused (wrong_issuer) without the token. A push while the JWKS is down gets 503 temporarily_unavailable, and the log says jwks_unavailable.

Summary by CodeRabbit
New Features
Readiness details and operational logs now identify failed token checks without exposing tokens or sensitive validation details in client-facing responses.
Failed bundle syncs report whether a cached bundle is still being served.
Bug Fixes
Push-token refusals use a consistent 401 response across validation failures; unavailable JWKS returns 503.
JWKS outages are distinguished from rejected tokens, while previously loaded bundles remain available when possible.
Documentation
Updated deployment and readiness guides with token-failure diagnostics and response behavior.
… or push token failed
An expired pull token, one from another issuer or for another audience, one
signed under a kid the JWKS doesn't publish, a bad signature and an unreadable
JWKS all ended in one generic reason, so operators couldn't tell them apart.
JwksService now reports which check refused a token (VerifyResult.failure, from
the error jose throws), and the saas source adds wrong_audience and
jwks_unavailable. The source logs the check a pull token failed at warn level
with its id, never the token; the sync outcome and the /readyz check carry it as
lastSync.tokenFailure; a push logs it; a Worker's failed Cron run names it.
What callers see is unchanged: a refused push token still answers the generic
401 invalid_token, a push while the JWKS can't be read still answers 503
temporarily_unavailable, and the tools' SKILL_BUNDLE_NOT_LOADED reason keeps its
text.
The change adds typed token-verification failure categories and carries them through SaaS bundle pulls, synchronization outcomes, readiness details, and push-token handling. Tests and documentation cover these diagnostics, including the distinction between rejected tokens and unavailable JWKS.
JWKS verification results now include typed failure categories for token refusals and unavailable keys. Tests cover provider verification and weak-key fallback classifications.
Propagate failures through the SaaS source libs/adapters/src/skills/sources/*, libs/adapters/src/skills/index.ts, libs/adapters/src/skills/__tests__/*
The SaaS source distinguishes token rejection from JWKS unavailability. It carries failure categories through pull errors, push refusals, cached bundle origins, and pull-failure listeners.
Record failures in sync and readiness outcomes plugins/plugin-skilled-openapi/src/sync/*, plugins/plugin-skilled-openapi/src/__tests__/*, libs/edge/src/*, libs/edge/README.md, libs/skills/catalog/*
Synchronization outcomes and readiness details now include token-failure information when available. Tests and documentation describe failed loads, cached-bundle fallback, and operator-facing details.
Handle push refusals and verify endpoint behavior plugins/plugin-skilled-openapi/src/push/*, plugins/plugin-skilled-openapi/src/__tests__/bundle-push.*, apps/e2e/demo-e2e-skilled-openapi/*
Push-flow logs include classified failures when available. Tests cover generic 401 responses for refused tokens, 503 responses for unavailable JWKS, and the corresponding endpoint behavior.

Startup diagnostics can omit the JWKS failure reason. In the narrower encryption-only key case, startup can also reject a pull instead of serving a cached bundle. Fix these before merging if those operational paths must be reliable.

…d JWKS outage keeps its reason
A key whose key_ops leaves out verify passed the usable-key check though jose won't verify with it, so a token got unknown_key in place of jwks_unavailable. tokenFailureOf now reads an error's cause, so a start with no cached bundle reports jwks_unavailable too.

What and why
The
saassource checks tokens against the issuer's JWKS: the pull token before every pull, and the token of each push. Until now, every failed check ended in one generic reason:not a valid token from issuer "…" signed by a key in … (no_provider_verified). An expired token, a token from another issuer, one signed by a key the JWKS doesn't publish, a bad signature: all read the same. What the verifier knew went only to a debug log. Operators couldn't tell from the logs or/readyzwhich check failed. Each failed check now has a name,

@frontmcp/auth:JwksService.verifyTransparentTokensets a newfailureon a refusal, from the errorjosethrows (itscode, and the claim for a claim check). The weak-RSA fallback sets it too. With several providers, it names the check of the provider that got furthest: a claim check means that provider's key verified the signature. Tests
Added:
@frontmcp/auth,jwks.service.failure.spec.ts: eachfailure, from tokens signed with test keys:expired,not_yet_valid,wrong_issuer(anotheriss, and none),missing_claim,unknown_key,bad_signature,malformed(not a JWT, and anexpthat is a string),unsupported_algorithm(HS256),jwks_unavailable. Also: with two providers, in either order, the one that got furthest names it; an accepted token has none; and the weak-RSA fallback namesexpired,not_yet_valid,wrong_issuer,missing_claimandbad_signature.jwks.service.missing-exp.spec.tsnow expectsfailuretoo.@frontmcp/adapters:saas-pull.source.verification.spec.ts: a refused pull token carries each reason,wrong_audienceforaudandresourceincluded. The message clients read doesn't name it. The warn line has the source id and no token. An unreachable JWKS isjwks_unavailable, and so is one that answers 503, isn't JSON, or has no keys or no usable key.saas-pull.source.spec.ts: the cached bundle served because the JWKS was down carriestokenFailure: 'jwks_unavailable'. The pull failure listeners get the token check a poll failed on (unknown_keyafter a key rotation), and none for a 502.saas-pull.source.push.spec.ts: each push refusal carries its reason (wrong_audience,wrong_issuer,expired,missing_claim,bad_signature,unknown_key), and the replay guard's refusals carry none. The message names it, an unreadable JWKS throwsSaasJwksUnavailableError, andtokenFailureOfreads each.bundle-sync.service.spec.ts: the outcome names the token check on a start that loads no bundle, a refresh, a poll, a start on the cached bundle (jwks_unavailable), and a refresh whose JWKS can't be read. A pull that fails for another reason has none.bundle-load-state.spec.ts: a stale check carrieslastSync.tokenFailure, and an unhealthy one carrieslastSync.bundle-push.flow.spec.ts: a token refused asexpired,wrong_issuer,wrong_audience,unknown_key,bad_signatureormalformedanswers the same 401, and the answer doesn't name the check while the log does. An unreadable JWKS answers 503 and logsjwks_unavailable.bundle-push.integration.spec.ts(a real server): four tokens refused by different checks get the same 401 body.last-good-bundle.integration.spec.ts(a real server, read through the SDK's health service): a pull token from another issuer makes the checkunhealthywithlastSync.tokenFailure: 'wrong_issuer', anderrordoesn't name it. A refresh whose JWKS can't be read isfailedwithjwks_unavailable, and the check isdegraded.index-wiring.spec.ts: a Cron run whose pull token expired, or whose JWKS couldn't be read while the cache serves, throws with(token check: …).demo-e2e-skilled-openapi, saas on Node:kidshowswrong_audience,wrong_issuer,expired,bad_signatureorunknown_keyin/readyz'slastSync. The log has the warn line and not the token. The tools' reason doesn't name the check.jwks_unavailablewhile the cache serves.401 invalid_token, and the log sayspush refused (wrong_issuer)without the token. A push while the JWKS is down gets503 temporarily_unavailable, and the log saysjwks_unavailable.Run (after merging #1129):
