### Corporate TLS inspection and proxy block pages

If every command exits `10` with a message naming `NODE_EXTRA_CA_CERTS`, the
machine does not trust the certificate chain presented by the network. Node's
default trust store is its bundled Mozilla roots plus any PEM bundle named by
`NODE_EXTRA_CA_CERTS`; it does not read the operating-system store. Point that
variable at the corporate root CA (for example
`NODE_EXTRA_CA_CERTS=/path/to/corporate-root-ca.pem`). Never set
`NODE_TLS_REJECT_UNAUTHORIZED=0` — it disables certificate checks for requests
that carry the API key.

If a proxy, firewall, or captive portal answers with an HTML `4xx` page, the CLI
reports `UNAVAILABLE` with `details.reason: "not_testsprite"`, plus the status,
content type, and page title, instead of mapping the status to an API verdict
such as `AUTH_FORBIDDEN`. Ask IT to allow the API host, and set `HTTPS_PROXY` /
`HTTP_PROXY` when the network requires an explicit proxy. A JSON API error
envelope still maps by status; `408`, `429`, and `5xx` keep their existing retry
behavior.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Stop marking an unclassified TransportError as reachedApi: true.

classifyFetchFailure can return undefined for an unrecognized code such as EHOSTUNREACH. In that case src/lib/http.ts Line 940 throws new TransportError(message, requestId) with empty details. isTransportFailure then returns false, and Line 360 reports reachedApi: true. checkLocalTunnel runs the probe, the probe fails with UNAVAILABLE, and Line 469 prints "not available on this endpoint". The PR intends to remove exactly that diagnosis. The request never produced an HTTP response, so it never reached the API. TransportError from an edge 502/504 is a different case because a response did arrive.

Use the HTTP status as the reachability signal. Every response-derived ApiError has httpStatus, and fetch-rejection TransportErrors do not.

Review comment at @src/commands/doctor.ts around lines 352 - 361:
Update the fallback error handling in classifyFetchFailure so reachedApi is
determined by whether error.httpStatus is defined, rather than always being
true. Preserve reachedApi: false for isTransportFailure errors and ensure
response-derived ApiError instances remain true while fetch-rejection
TransportError instances remain false.
