Why
The review route follows signed importance even when a repository requires PR review for every change.
Changes
Read an optional owner-local repository policy that selects PR review at every importance level.
Keep the existing routes when the policy is absent, and reject malformed or mismatched policies.
Preserve signed assessments, prior findings, current-head review requirements, and separate owner-controlled pauses.
Require completed PR review before readiness under the repository policy.
Add routing and CLI regressions, proof obligations, updated review guidance, and an ADR.
Regenerate the ADR index and proof counts with #456 as the base.
Closes #457.
