Summary

Addresses part 2 of #5920: reuse the existing fixed guardrail-blocked message when input validation rejects chat, achat, or streaming start. Ordinary non-streaming start delegates to chat. Managed-backend start validates before direct delegation, without calling chat again. Planning validates the initial request before creating a plan and each generated step through chat, then stops immediately on rejection. Caller-mode start/astart and canonical return_outcome preserve failure. start(prompt=None) resolves instructions or Hello before validation.

Previously a rejected input returned None from non-streaming entry points, violating the existing string contract (result.strip() crashes). The stream path emitted validator diagnostics instead of the established fixed message. The fixed text carries a private str subtype marker so the shared sync/async team-result processor rejects blocked results before memory, task-output, file or completion processing. Direct callers still receive usable string text. No message-text matching or mutable agent-level rejection flag is used. No new message, configuration, or public API is introduced. This does not resolve the persistence portion of #5920. The independent async-loop fix is #5932.

Validation

  • Four public entry-point regressions fail on main. Two further public team regressions fail on the original PR head (blocked work incorrectly completed); all pass after correction. An ordinary identical-text control still completes.
  • 57 passed / 1 skipped across input rejection, existing guardrail object/message tests, and DB-run lifecycle tests (Windows/Python 3.13).
  • Real Agent.start and Agent.achat with an accepting validator both returned generated text from an existing local pretrained stories15M model through an owned loopback llama.cpp server. Exact server process stopped. The rejected branch itself is validated offline before any provider dispatch.
  • Another 15 task-guardrail/structured-policy tests pass.
  • New test Ruff and diff-check pass; full suite not run.

Summary by CodeRabbit

  • Bug Fixes

    • Guardrail-rejected input now returns a consistent blocked message across synchronous, asynchronous, and streaming interactions.
    • Rejected team tasks are marked as failed and do not save output or invoke completion callbacks.
    • Planning stops when the initial prompt or a later step is blocked.
    • Caller-mode results correctly report guardrail rejections as unsuccessful, while ordinary responses matching the blocked-message text remain successful.
    • Managed-backend requests are checked before delegation, including streaming requests.
  • Tests

    • Added coverage for blocked responses across supported execution paths.

Summary

Returns the existing blocked message as usable text while carrying a private rejection marker.

  • Rejected prompts return the fixed guardrail message across agent entry points.
  • Planning stops when its request or a generated step is rejected.
  • Caller-mode and canonical results keep guardrail rejections marked as failures.
  • AgentTeam marks guardrail-blocked work failed before saving or completing it.

@claude You are the FINAL architecture reviewer. If the branch is under MervinPraison/PraisonAI (not a fork), you are able to make modifications to this branch and push directly. SCOPE: Review changes in this PR. Python SDK: praisonaiagents, praisonai. TypeScript SDK: src/praisonai-ts/. Do NOT modify src/praisonai-rust. Read ALL comments above from Gemini, Qodo, CodeRabbit, and Copilot carefully before responding.

MANDATORY READ (before reviewing):

  • Always read src/praisonai-agents/AGENTS.md
  • If this PR touches src/praisonai-ts/, also read src/praisonai-ts/AGENTS.md §2.1.2 (TS triage + PR review checklist)

Phase 1: Review per AGENTS.md

  1. Protocol-driven: check heavy implementations vs core SDK
  2. Backward compatible: ensure zero feature regressions
  3. Performance: no hot-path regressions
  4. SDK value: review in depth whether the change genuinely adds value to the SDK — never add features for the sake of adding them. It must strengthen the SDK (simpler, more user-friendly, robust, world-class, secure). If it does not clearly add value, request changes or recommend rejecting/closing rather than merging scope creep
  5. Do not bloat the Agent class with additional params — only if absolutely required; we already support many params.
  6. Repo routing: agent-callable tools → PraisonAI-Tools; lifecycle plugins → PraisonAI-Plugins; optional sandbox backends → PraisonAI-Plugins (praisonai.sandbox entry point) — request changes if wrongly added to praisonaiagents/
6b. External tool examples (examples/tools/external/**): If the diff is confined to that path, mark SDK value ❌ for monorepo merge — redirect the contributor to PraisonAI-Tools; do not Approve for merge unless the PR has the maintainer-accept-example label.
6c. New vendor *_tools.py in core (src/praisonai-agents/praisonaiagents/tools/): If the PR adds a new *_tools.py for a third-party API/SaaS/dataset (e.g. FXMacroData), mark Repo routing ❌ and SDK value ❌ — redirect to MervinPraison/PraisonAI-Tools. Do not Approve because legacy bundled tools (Tavily/Exa/You.com/etc.) already in core are not precedent for new vendor modules. Request changes or recommend closing unless the PR has maintainer-accept-core-tools.

MANDATORY COMMENT FORMAT — include this Phase 1 table in your review comment:

Phase 1 — AGENTS.md review

Check Result
Protocol-driven / no heavy impl in core ✅ or ❌ + one-line rationale
Backward compatible ✅ or ❌ + one-line rationale
Performance (hot path) ✅ or ❌ + one-line rationale
SDK value ✅ or ❌ + one-line rationale (explicitly judge whether the change strengthens the SDK)
No Agent param bloat ✅ or ❌ + one-line rationale
Repo routing ✅ or ❌ + one-line rationale

For TypeScript PRs (src/praisonai-ts/), also add:
| TS types / parity / tests | ✅ or ❌ + one-line rationale (npm run build && npm test) |

Phase 2: FIX Valid Issues
7. For any VALID bugs or architectural flaws found by Gemini, CodeRabbit, Qodo, Copilot, or any other reviewer: implement the fix
8. Also independently identify and fix any gaps or issues you find in the changed code — do not rely only on prior reviewer feedback
9. Push all code fixes directly to THIS branch (do NOT create a new PR)
10. Comment a summary of exact files modified and what you skipped

Phase 3: Final Verdict
11. Never Approve or write “Approve for merge” if any Phase 1 row is ❌, if rule 6b/6c applies, or if the change belongs in PraisonAI-Tools/PraisonAI-Plugins. Use Request changes, redirect to the external repo, and recommend closing the monorepo PR when routing is wrong.
12. Approve only when every Phase 1 row is ✅ (including Repo routing and SDK value) and blocking issues are resolved; otherwise request changes with clear action items

Security review: PASS

Advisory only. A maintainer merges fork PRs. This review is based on the PR data alone: diff, description and source issue #5920. Nothing from the fork was run.

Checklist

  1. Scope: OK. The PR changes 2 files (+34/−3). It makes three one-line changes in agent/chat_mixin.py and adds one new unit test. Both serve part 2 of Input guardrail blocks the asyncio loop and returns None on rejection; learned policy rules and the permission allowlist lose or corrupt data on concurrent or interrupted writes #5920 only. It does not touch persistence or the async offload (fix: keep synchronous input guardrails off the async event loop #5932 covers that).
  2. Exfiltration / network: None. No new URLs, hosts or network calls. The test uses llm="gpt-4o-mini", but every case is rejected before any provider is called.
  3. Code execution: None. No eval/exec, subprocess, dynamic imports or deserialization.
  4. Secrets: None read or written. This PR actually reduces leakage. The stream path used to yield f"[Input blocked by guardrail: {_in_err}]", which put the validator's free text into user output. It now yields the existing fixed message. The reason still goes to logging.warning as before, so logging is unchanged.
  5. Weakened controls: None. Rejected input still returns or yields before the prompt is reassigned or dispatched. The guardrail is still enforced; only the return value changes, from None (or the diagnostic string) to the fixed blocked message.
  6. Supply chain: No changes to dependencies, manifests, CI, conftest or entry points.
  7. Obfuscation: None. The diff is short, readable ASCII.
  8. Tests: Nothing removed or loosened. The new test checks equality against agent._guardrail_blocked_message() and also checks the result is non-empty. Both assertions would fail on main (None, or the leaking diagnostic string).
  9. Backdoors / magic values: None.
  10. Injection artifacts: None in the code, comments or test data. The PR body is a plain description with no text addressed to AI tools or reviewers.

Deterministic risk flags: none were provided with this request, so there are none to address.

Code review

  • The fix is the minimal one Input guardrail blocks the asyncio loop and returns None on rejection; learned policy rules and the permission allowlist lose or corrupt data on concurrent or interrupted writes #5920 asked for. It reuses the existing _guardrail_blocked_message helper, adds no new params, exports or config, and adds 0 lines to agent.py. That fits AGENTS.md's "lightweight" aim.
  • API behaviour change: callers that checked if result is None to detect a blocked input will no longer see None. This matches the output-guardrail contract (Guardrail block returns None instead of safe user message #5596), and the reason is still recorded. It's worth mentioning in release notes.
  • Not verified here: whether _guardrail_blocked_message takes an optional reason and records it in last_guardrail_error. The diff calls it both as _guardrail_blocked_message(_in_err) and with no arguments (in the test). Please check the helper's signature on main before merging. If it records the reason, the stream path no longer leaks it but it stays available to code.
  • Test nit: asyncio.run(agent.achat(...)) inside a sync parametrised test is fine. If the repo runs pytest-asyncio in strict mode, an async def test would be more idiomatic. Not blocking.
  • CI: merge_state=UNSTABLE and the PR is labelled pipeline/blocked:ci. Please confirm CI is green on head 783262a before merging. The author ran 54 targeted tests, not the full suite.

Verdict: LGTM once CI passes and the helper signature is checked.

Reproduced the team-task regression through both run_task and arun_task: on the original PR head a rejected input was marked completed and persisted as output. Corrected in 41f16a10f.

The existing fixed text now carries a private str subtype marker; direct users still receive nonempty usable text. The shared task-result processor checks the marker before memory/result/output/completion processing and preserves failure. No message-text matching or mutable per-agent rejection flag was added. A control confirms ordinary text identical to the apology still completes normally.

Both new public team regressions fail before correction; 57 related tests pass / 1 skip, plus 15 task-guardrail/structured-policy tests. New-test Ruff and diff-check pass. Fresh committed-head real local Agent.start/achat return generated text with an accepting validator; owned server stopped. Full-suite/remote CI remains unconfirmed.

Stop failed workflow tasks before routing next_tasks.

_process_task_result now marks a guardrail-rejected task as failed and returns no output. After the workflow yields that task, Process.workflow() still selects and yields its first next_tasks entry without checking the failed status. A rejected task can therefore execute its dependent even though the default on_error="stop" applies. The routing must allow this only when the failure or dependent explicitly configures continuation.

Suggested fix
             if not next_task and current_task and current_task.next_tasks:
                 next_task_name = current_task.next_tasks[0]

Check failed workflow predecessors before async dispatch.

_deps_failed(task_id) already recognizes a next_tasks predecessor through task.previous_tasks. However, arun_all_tasks() checks it only when _depends_on_pending() flushes an async batch. A rejected synchronous predecessor leaves no pending batch, so its default-stop successor reaches run_task() and executes. Apply the guard after any required flush in both scheduler branches. Preserve _should_continue_on_dep_failure() so configured continuation still runs.

This async scheduler change is separate from the synchronous Process.workflow() routing correction.

                     # If this async task depends on another async task still

Confirmed both workflow-routing observations through public start/astart calls, then repeated with an ordinary None result on unchanged main: the failed predecessor still dispatched its successor before this PR existed. The old None-result branch already set task.status="failed", so this is a pre-existing scheduler gap rather than a regression from the string rejection marker.

I repaired that independent root in #5936, keeping this PR focused. The fix reuses the existing dependency-failure policy helpers at the AgentTeam dispatch boundary for sync and async scheduling, after any required batch flush. It does not add a second routing/failure-policy implementation in Process. Original matrix had 4 failures / 8 passing continuation controls; expanded final matrix plus existing context propagation gives 32 passes. Explicit dependent continue/skip controls remain intact.

The verdict is advisory. This is a fork PR and a maintainer has to merge it. I reviewed the full diff (5 files, +80/−4) at head 41f16a10, using only the PR data. I did not check out or run anything.

# Check Result
1 Scope Every change serves #5920 part 2: input-guardrail rejection now returns usable text. One effect goes further than the title, covered in note A below. It is related to the issue and not malicious.
2 Exfiltration / network None. No new URLs, hosts, sockets or telemetry. The tests reject input before any provider dispatch, or monkeypatch chat.
3 Code execution None. The only new imports are lazy, inside functions, from an internal module (..guardrails.guardrail_result). No eval/exec, subprocess, pickle or dynamic loading.
4 Secrets None read or logged. The stream path used to yield the validator's diagnostic ([Input blocked by guardrail: {_in_err}]) to the end user. It now yields the fixed message, so the change leaks less information.
5 Weakened controls None. Rejected input still returns early, before any LLM call. Blocked team tasks now fail where they used to be treated as output, so the control is stricter.
6 Supply chain No manifests, lockfiles, CI, conftest or entry points touched.
7 Obfuscation None. Plain, short lines and no binary files. I saw no bidi or zero-width characters.
8 Tests Additive only: one new test file, nothing removed or loosened. The negative control (test_plain_text_is_not_classified_by_its_message) checks that the fix doesn't just match on the message text.
9 Backdoors None. No magic values or user-, date- or host-keyed behaviour. The marker is a private str subclass, not a string match.
10 Injection artifacts None found in the code, comments, docstrings or PR body.

No deterministic risk flags were included with this request, so there was nothing extra to address.

Code review

Architecture: OK. agent.py gains only +4/−1. No new params, exports, dependencies or config. _GuardrailBlockedResponse is private, and the reused message text already existed. This fits the AGENTS.md rule to stay minimal.

Correctness: The contract fix is right. chat/achat returning None broke callers that call result.strip(). The stream path also differed from the output-guardrail message.

Notes for the maintainer:

A. Output-guardrail blocks also change behaviour in teams. _guardrail_blocked_message() is shared with the output-guardrail path. So once this lands, an output-blocked response in AgentTeam also becomes status="failed" with no task.result, no output_file and no on_task_complete. Before, the apology text was stored as a completed result. That is arguably the correct behaviour, and the new error string says "input or output". But the PR title and description only mention input, so please confirm it's intended. It deserves a test or a line in the changelog.

B. The marker can be lost. Any str operation (.strip(), f-string, +, str()) between chat() and _process_task_result turns it back into a plain str. The block would then fall back to being treated as a completed task. This only falls back to the old behaviour, so it's not a security problem, but it is fragile. Check that no hook or wrapper in the team chat → _process_task_result path normalises the output. I couldn't verify the callers from the diff alone.

C. Not tested: task.status = "failed" is set inside the shared processor, so check that the sync and async callers don't later overwrite it or start a retry loop. The new team tests assert status == "failed" on both paths, which suggests they don't, but I couldn't confirm it from the diff alone.

D. CI is not green on HEAD (pipeline/blocked:ci), and the PR says the full suite wasn't run. It should not be merged until the full suite passes in CI.

Verdict: The change is small, focused and safe. It needs a maintainer to confirm A and D (and ideally B and C) before merging.

Security review: PASS

Advisory only. This is a fork PR, so a maintainer has to merge it. I reviewed head cf7ac667 using only the PR data from the GitHub API. Nothing from the PR was checked out, run or imported.

Checklist

  1. Scope: OK. All 5 files serve Input guardrail blocks the asyncio loop and returns None on rejection; learned policy rules and the permission allowlist lose or corrupt data on concurrent or interrupted writes #5920 part 2: input-guardrail rejection returned None from chat, achat and start, and the stream path leaked diagnostics. The team-result change is the consumer side of the same fix. Note that it also changes behaviour for output-guardrail blocks (see review below). It is related, but it goes beyond "input".
  2. Exfiltration / network: None. No new URLs, hosts, sockets or telemetry.
  3. Code execution: None. No eval/exec, subprocess, pickle or yaml. The only dynamic part is two lazy relative imports of a private class from the package itself (..guardrails.guardrail_result).
  4. Secrets: None. No env, credential or file reads. The existing logging.info of the block reason is unchanged.
  5. Weakened controls: None. Blocking is just as strict as before, and every reject branch still returns early before any LLM dispatch. The stream path is stricter: the validator diagnostic (_in_err) is no longer sent to the end user.
  6. Supply chain: None. No manifests, lockfiles, CI, conftest or entry points are touched.
  7. Obfuscation: None. Short, readable lines. I saw no encoded blobs or unusual unicode in the diff.
  8. Tests: Nothing was removed or loosened. One new test file was added. The assertions are meaningful: task.status == "failed", task.result is None, the output file is not created, and the completion callback does not fire. The control test (identical plain text → completed) guards against string matching.
  9. Backdoors: None. No magic values, and nothing keyed on users, dates or hosts.
  10. Injection artifacts: None in the code, comments, docstrings or test data. The CodeRabbit HTML comments in the PR body are standard bot markers.

No deterministic risk flags were provided for this PR.

Code review

What it does: input rejection now returns the existing fixed _guardrail_blocked_message() text instead of None or a diagnostic. That text is now a private str subclass (_GuardrailBlockedResponse). _process_task_result uses isinstance on that class to mark the task failed instead of completing it. The change is small (+89/−4), touches agent.py by only +3 net lines, and adds no new params, exports or deps. That fits AGENTS.md.

Points to consider before merging:

  1. Behaviour change for direct callers. chat() and achat() used to return None on input rejection, so a caller could check if result is None or if not result to detect a block. Now they get a truthy string. Internal callers that used falsy-ness as the failure signal will now treat a block as success. Please check this before merging: grep for chat( and achat( callers that branch on None or falsy results, such as autonomy and iteration loops, the hierarchical manager, handoffs and workflows. The test only covers AgentTeam.run_task and arun_task.
  2. The marker breaks easily. Any str operation (.strip(), slicing, f-string formatting, +, json round-trip) turns the value back into a plain str and silently drops the marker. If any code between chat() and _process_task_result reformats the output (output_json/pydantic parsing, result wrappers or streaming aggregation in the task path), a blocked turn would again be recorded as "completed". It would help to add a comment at _guardrail_blocked_message stating that the result must be passed through unmodified.
  3. Output-guardrail scope. _guardrail_blocked_message is also used by the output-guardrail path, so output-blocked team tasks now become failed too. They were previously completed with the fixed text. That is arguably correct, and the test covers it, but it is a user-visible behaviour change. The PR title and changelog only mention input.
  4. Failed-task semantics. The early return sets task.status = "failed" and returns TaskResult(success=False, ...) without going through any retry or max_retries handling. That is reasonable for a deterministic input block. Please confirm that downstream workflow steps (sequential/hierarchical start() paths, not only run_task) handle task_output=None without crashing.
  5. Nit: the test uses llm="gpt-4o-mini" with no network. That is fine for the input-rejection cases (they return before dispatch), and the output cases mock llm_instance.

Verdict: the change is clean, minimal and well-tested for the paths it targets, and I found no security concerns. Before merging, the maintainer should check the impact on internal callers that relied on None (point 1) and the marker breaking (point 2). CI is currently not green on HEAD (pipeline/blocked:ci), and that also needs to be resolved.

Validate input before managed-backend dispatch.

Agent.start() directly calls _delegate_to_backend() when backend is configured. The PR updates rejection handling in chat(), achat(), and _start_stream(), but this start() route still bypasses input validation.

A rejected prompt can therefore reach the managed backend and be recorded as a normal backend response. Apply the guard before delegation and preserve the streaming return shape.

Stop planning when a step returns a guardrail-blocked response.

When self.chat(step_prompt, **kwargs) returns _GuardrailBlockedResponse, _start_with_planning records it as a completed step. It also adds the blocked text to context and to all_results_text. The compilation call then runs self.chat(compilation_prompt, **kwargs). That call is validated as a new input. If it passes, start can return ordinary generated text instead of the standardized blocked response. The single-step path returns the blocked response, so the two paths behave differently.

Add an early return after the step call:

Validate input before planner dispatch.

With planning enabled, Agent.start() sends the original prompt to PlanningAgent.create_plan_sync() before the executing agent runs its input guardrail. The no-tools planner path calls llm.get_response() directly.

A rejected prompt can therefore reach the planning model and continue through a valid plan instead of returning the standardized blocked response. Apply the same input validation before creating the plan.

Handled review 5643058025 in the original branch: managed-backend start validates before delegation and preserves its streaming shape; planning validates before planner dispatch and returns immediately after a blocked step rather than compiling it; caller-mode start/astart preserve rejection as success=False with the safe message in error. Added rejected/accepted backend controls, validated-prompt forwarding, stopped-versus-completed multi-step planning, and sync/async caller-mode controls. The original focused failures were reproduced, including the async parity gap after the initial sync correction; all 45 related tests now pass. Actual local pretrained inference through Agent.start, achat and an AgentTeam workflow also returned text; the owned inference server was stopped. This extends the existing typed rejection contract without introducing public configuration or a new policy. No CI/merge claim.

Security review: PASS

Advisory only. A maintainer has to merge this because it is a fork PR. I read the PR data alone (head 3d1d00f, 6 files, +186/−7). I ran nothing and checked nothing out.

Risk flags: this request listed no deterministic risk flags. My own pass over the added lines found none either.

# Check Result
1 Scope Mostly OK. Every change is about issue #5920 part 2 (an input rejection should return the fixed blocked string and must not count as a success). There are two extra edits that the PR body does not mention: input validation in the backend delegate path of start(), and input validation in _start_with_planning. Both are related and tighten control, because those paths used to skip the input guardrail. Users of backend= or planning will see a behaviour change, so the PR body should say so. Not counted as a finding.
2 Network / exfiltration None. No URLs, hosts, sockets or telemetry.
3 Code execution None. No eval/exec, subprocess, dynamic import or pickle/yaml. The only new imports are lazy relative imports of ..guardrails.guardrail_result._GuardrailBlockedResponse.
4 Secrets None. No new env, credential or file reads. This removes a leak: the stream path used to yield f"[Input blocked by guardrail: {_in_err}]", which put the validator's free text into user output. It now yields the fixed message. AutonomyResult.error carries only the fixed text, not the reason.
5 Weakened controls None. The input guardrail now runs on more paths (backend delegate, planning, each planning step). Blocked team tasks are marked failed, skip memory storage and output-file writes, and do not fire the completion callbacks. The marker is a str subclass. If something transforms the string (.strip(), concatenation) the marker is lost, and the result is treated as an ordinary success. That is the same as current behaviour, so it is not a regression.
6 Supply chain No changes to dependencies, manifests, CI, conftest or entry points.
7 Obfuscation None seen. The diff is short readable Python with no blobs, long lines or odd unicode.
8 Tests Only a new test file. No existing tests were removed or loosened. The tests cover both outcomes: blocked vs normal, and an ordinary response with identical text still completes, so they would not pass if the fix were missing.
9 Backdoors None. The only fixed value is the existing blocked-message string. There is no check that skips validation based on a user, host or date.
10 Injection artifacts None in code, comments, docstrings or test data.

Code review

Architecture: this is core SDK work and belongs in praisonaiagents. agent.py changes by +4/−1. It adds no new public API, parameter or dependency. The marker class is private, and it lives next to GuardrailResult in guardrails/guardrail_result.py. That is lightweight and fits the issue.

What's good

  • chat, achat and stream now honour the documented contract (Guardrail block returns None instead of safe user message #5596) that a blocked turn always returns a str and never None.
  • Blocked results are identified by type, not by matching the message text. test_plain_text_is_not_classified_by_its_message checks this.
  • Caller-mode start/astart now return success=False for a block. Before, they reported success=True.

Points to check before merge

  1. Possible double validation on the backend path. start() now validates and then calls _delegate_to_backend(prompt). If the delegate (or the backend it calls) goes through chat() again, the input guardrail runs twice. For an LLMGuardrail that means two moderation calls, and a rewriting guardrail would be applied twice. Please confirm the delegate does not re-validate.
  2. start(prompt=None) with a backend. The new _validate_input_with_guardrail(prompt) call runs before any check that prompt is set. Please confirm the helper and the user validate_input implementations accept None.
  3. Planning steps are validated twice. Each self.chat(step_prompt) now runs the input guardrail on text the planner generated. A false positive on any step now aborts the whole plan. That is probably intended, but it changes behaviour and should be documented.
  4. The marker is fragile. Any wrapper or hook between _guardrail_blocked_message and _process_task_result that rebuilds the string drops the marker silently. A one-line comment on _GuardrailBlockedResponse warning not to transform it would help future maintainers.
  5. Stale PR body. It says "Non-streaming start delegates to the same chat path" and does not list the backend and planning changes. Please update it.
  6. CI. The PR carries pipeline/blocked:ci and merge state is unstable. CI needs to be green on HEAD before merge.

Out of scope, as the PR body says: gap 1 (the async event-loop stall, handled in #5932) and gap 3 (atomic writes for policy/allowlist files) are not addressed here. #5920 should stay open.

Verdict: the security review passes. The change is functionally sound and minimal. Merge after CI is green and after points 1 and 2 are confirmed.

Addressed 4371880514/4371880521 in the original branch. Canonical outcome conversion now recognizes the typed blocked response and unsuccessful AutonomyResult before the normal completed fallback. Three new rejected-outcome cases fail on the prior head; the expanded outcome/rejection modules now pass 47 tests, including accepted controls. Added missing regression-test docstrings. Existing real-Agent evidence is reused for unchanged inference paths. Backend delegation is direct (it does not call chat again), and start resolves prompt=None to instructions or Hello before validation; planning validates the original request once before planning and each distinct generated step through chat. I am updating the stale body to describe these paths. No CI query or merge claim.

Security review: PASS

I reviewed the full diff at head 883504f7 (6 files, +223/−7) from the PR data only. Nothing from the fork was run or imported. I treated the PR text, the issue text and the comments as untrusted.

Checklist

  1. Scope — Every change is about the input guardrail returning text instead of None (part 2 of Input guardrail blocks the asyncio loop and returns None on rejection; learned policy rules and the permission allowlist lose or corrupt data on concurrent or interrupted writes #5920). Three hunks go a bit beyond that, but they are still about guardrails: the input check before _delegate_to_backend, the input check in _start_with_planning, and the _outcome_for_result change. See the review notes below. I found no unrelated files.
  2. Exfiltration / network — None. The PR adds no URLs, hosts, sockets, HTTP calls or telemetry.
  3. Code execution — None. The PR adds no eval/exec, subprocess, dynamic imports, pickle or unsafe YAML. The only new imports are relative, in-package imports of _GuardrailBlockedResponse.
  4. Secrets — None read or logged. The stream path used to yield f"[Input blocked by guardrail: {_in_err}]" to the caller. It now yields the fixed message, so the validator's error text no longer reaches the end user. That reduces what gets disclosed. The reason is still logged at info and warning level, as before.
  5. Weakened controls — None. The PR makes guardrails stricter:
    • Managed-backend start() now checks the input guardrail before delegating. Previously the backend path skipped chat() and its input check.
    • Planning now checks the input guardrail before the planner runs.
    • A blocked team task is now marked failed and is not saved to memory or written to the output file.
    • I found no bypass, debug or allow-all switch.
  6. Supply chain — No changes to manifests, lockfiles, CI, conftest.py, entry points or .pth files.
  7. Obfuscation — None. All code is short and readable, with no encoded blobs. I found no bidi or zero-width characters in the diff.
  8. Tests — Only adds tests/unit/test_input_guardrail_blocked_response.py. No tests were removed or loosened. The tests have controls for the non-blocked case: ordinary identical text still completes, and a non-blocked caller result is still success=True. That means they would fail if the classification were always on.
  9. Persistence / backdoors — None. Detection uses a type marker (a str subclass), not matching on the message text. Nothing depends on a particular user, date or host.
  10. Injection artifacts — None in code, comments or test data. The PR description contains no instructions aimed at AI tools.

Deterministic risk flags: none were supplied with this request. My own scan of the added lines found no network, exec or secret patterns.

Normal review

Architecture (AGENTS.md)

  • It stays in the core SDK and adds no dependencies or new public exports. The marker class is private (_GuardrailBlockedResponse).
  • agent.py grows by only 3 lines, and no new Agent params are added. The fix fits the "lightweight" rule.

Correctness

  • The fix itself is right. chat/achat used to return None on rejected input, which broke the string return contract (result.strip() would crash). Reusing _guardrail_blocked_message() makes rejected input consistent with rejected output.
  • The managed-backend and planning paths were real gaps where blocked input was never checked. Closing them is worthwhile.

Points for the maintainer

  1. _outcome_for_result covers more than guardrails (execution_mixin.py). The new branch isinstance(result, AutonomyResult) and not result.success → RunOutcome(reason="failed") applies to every unsuccessful AutonomyResult, not only guardrail blocks. Any existing path that returns success=False with return_outcome=True would change from its current outcome to failed. Please confirm that is intended, or narrow the check to the caller-mode result.
  2. The marker is lost easily. _GuardrailBlockedResponse survives only while the object is passed along unchanged. str(), .strip(), f-strings, slicing or a JSON round-trip all turn it back into a plain str. Any path that transforms the output before _process_task_result would fall back to the old behaviour, where the blocked result is treated as success. That is not a regression versus main, but it is fragile. A short comment on the class saying "do not transform before checking" would help.
  3. Behaviour change for None checks. Callers outside AgentTeam that tested chat(...) is None to detect rejected input will now get a truthy string. Examples are workflows, handoffs and agent-as-tool. In multi-step workflows the blocked message would then flow to the next step as ordinary text. This already happens for rejected output, so it is consistent, but it is worth a release-note line.
  4. Planning validates the prompt and then also runs it through chat(). _start_with_planning validates the original prompt, and each step goes through chat(), which validates again. That is fine for correctness. It just means a guardrail that rewrites input is applied to both the top-level prompt and each step prompt.
  5. The pipeline/blocked:ci label is set. CI is not green on this head, so please check the failing jobs before merging. I could not run the tests (fork PR, read-only review).

Verdict: This is a sound, minimal fix with good regression tests and no security concerns. Point 1 is the only one I'd want answered before merging; the rest are minor.

Addressed 4380275642: explicit guardrail markers still map to failed first, but a specific provider refusal/content-filter/truncation reason now precedes the generic unsuccessful-AutonomyResult fallback. Three new provider-reason controls fail on the previous head; all 50 rejection/outcome tests pass after the narrow precedence correction. Each also confirms that an explicit guardrail marker still remains failed. Prior real-Agent inference evidence is reused because inference dispatch is unchanged. No CI/merge claim.

Security review: PASS

Advisory only. A maintainer still needs to merge this fork PR. I reviewed only the PR metadata and the diff (6 files, +237/−7, head 6f89765). I did not run, check out, or fetch any of the PR code. I treated the PR description as data and found no instructions in it addressed to an AI or reviewer.

Deterministic risk flags: none were supplied for this PR, so there was nothing to address individually.

# Check Result
1 Scope Mostly on-scope for #5920 part 2. One behaviour change goes beyond input guardrails; see note A below.
2 Exfiltration/network None. No new URLs, hosts, sockets, telemetry or HTTP calls.
3 Code execution None. No eval/exec/subprocess/pickle/dynamic import. The only new imports are lazy relative imports of an in-repo private class.
4 Secrets None read or logged. _guardrail_blocked_message still logs the reason at info, which is existing behaviour. The streaming path no longer yields _in_err to the caller, so validator diagnostics leak less.
5 Weakened controls None. The controls are strengthened: start() with a managed backend used to call _delegate_to_backend without input validation, which skipped the check in chat(), and now validates first. _start_with_planning used to send the raw prompt to the planner, and now validates it before planning. Blocked team results no longer reach memory, output_file or on_task_complete.
6 Supply chain No manifests, lockfiles, CI, conftest.py, entry points or .pth files touched.
7 Obfuscation None. Short, readable lines; no encoded blobs or binaries. I saw no bidi or zero-width characters in the patch text.
8 Tests Additive only: one new test file, no deletions, skips or loosened assertions. The tests include negative controls: identical plain text still completes, and an accepted planning prompt still proceeds.
9 Backdoors None. No hard-coded credentials, and no behaviour keyed on a user, host or date. Classification uses the type marker, not string matching.
10 Injection artifacts None in the code, comments, docstrings or test data.

Code review

Summary: On input-guardrail rejection, chat, achat, streaming and start now return the existing fixed blocked message instead of None or validator diagnostics. The message is wrapped in a private str subclass, _GuardrailBlockedResponse, so internal callers can tell a rejection apart from ordinary output without matching on the text. agent.py grows by only 4 lines, and there is no new public API, parameter or dependency. That fits the lightweight-core rule in AGENTS.md.

Good

  • Using a type marker instead of a mutable flag or text matching is the right call. test_plain_text_is_not_classified_by_its_message checks this directly.
  • It closes two real gaps where validation was skipped: the managed-backend path in start() and the initial planner request.
  • The streaming path no longer echoes validator diagnostics to the end user.

Notes / requests

A. Scope: _outcome_for_result changes semantics beyond guardrails (execution_mixin.py). The new branch isinstance(result, AutonomyResult) and not result.success → RunOutcome(reason="failed") applies to every unsuccessful AutonomyResult, not only guardrail blocks. Today such results come back as completed. That is arguably more correct, but it is a public behaviour change for return_outcome=True callers of autonomy runs (for example, hitting max iterations). It also overlaps with the recent #6064, which made the completion promise the only autonomy success signal. Please either restrict the branch to guardrail rejections or have a maintainer confirm it is intended and matches #6064. Ideally add a non-guardrail test for it.

B. Output-guardrail blocks now fail team tasks too. _guardrail_blocked_message is shared, so output-stage blocks now make _process_task_result return success=False instead of saving the apology text as task output. The tests deliberately cover this and I think it is correct, but it changes behaviour for existing teams. It should be called out in the release notes.

C. _process_task_result error message is generic ("input or output was blocked"). That's fine, and better than leaking the reason. Please confirm that the retry and max_retries handling in the team loop treats success=False here as terminal, not as a retry trigger. Otherwise a deterministic input block would just be retried.

D. Minor: _start_with_planning validates the original prompt, then each step goes through chat(), which validates again. That's correct, since the step prompts are different text. Just note that a guardrail which rewrites prompts (for example redaction) will now be applied to both the plan request and each step.

E. Minor: in caller mode, error is set to the user-facing apology string. A short constant such as "guardrail_blocked" would be easier for callers to branch on, but this is optional.

CI: labels show pipeline/blocked:ci and the merge state is UNSTABLE, so CI needs to be green on the head commit before merging. Per the PR description, the full test suite was not run.

Verdict: No security concerns. The change is small and well tested. Please resolve or confirm note A (and ideally C) before merging.

Addressed 4385593276 and review6434978287 A/C. Caller-mode wrappers now retain the private blocked-string marker in their output, so canonical conversion recognizes a current guardrail rejection before any stale provider reason. Generic unsuccessful autonomy results retain the pre-PR conversion contract; provider reasons remain authoritative for those generic results. No text matching or new public reason was added. The stale-reason and generic-contract cases fail on the prior head and now pass. The requested retry check also exposed four real sync/async input/output cases: a terminally failed result was reset and dispatched three times. Task runners now respect the failed status returned by result processing rather than retrying it. Expanded rejection/outcome/task-guardrail tests:57passed, including one-dispatch assertions with max_retries=3. Actual local Agent.start/achat/Team inference returned text; owned server stopped. Release note: output-guardrail blocks now fail team tasks rather than persisting an apology as success; original planning requests and generated steps each undergo their own validation. No CI or merge claim.

| TS types / parity / tests | ✅ or ❌ + one-line rationale (npm run build && npm test) |

I reviewed only the PR data from the GitHub API, at head bf1518e3 (6 files, +259/−9). Nothing from the fork was run or imported. The PR description is long and reads like an agent wrote it, but it contains no instructions aimed at a reviewer or AI. I treated it as untrusted and took nothing from it on trust.

# Check Result
1 Scope Mostly on target (see the scope note below). All changed files are in guardrail, agent-entry-point or team-result code, plus one new test file. Nothing unrelated.
2 Exfiltration / network None. No new URLs, hosts, sockets, HTTP calls or telemetry.
3 Code execution None. No eval/exec, subprocess, pickle, yaml or dynamic imports. The only imports added are relative, lazy imports of ..guardrails.guardrail_result and .autonomy, both inside this package.
4 Secrets None. No env vars, tokens or credential files are read. Logging is unchanged.
5 Weakened controls None. The guardrail gets stronger: (a) start() with a managed backend used to call _delegate_to_backend directly and skip input validation. It now validates before delegating. (b) _start_with_planning now validates the request before calling the planner. (c) Team runs no longer turn guardrail-blocked text into a completed task, saved output_file, memory entry or on_task_complete callback. (d) The stream path no longer echoes the validator's own error text to the caller ([Input blocked by guardrail: {err}]). It returns the fixed message, so less internal detail leaks.
6 Supply chain None. No manifests, lockfiles, CI, conftest, entry points or .pth files changed.
7 Obfuscation None. Lines are short and readable, with no encoded blobs and no unusual unicode.
8 Tests Additive only: one new file with 13 test functions (parametrized into more cases). No tests removed or loosened. They include negative controls (a plain string identical to the blocked message still completes, and generic autonomy failures keep their existing completed outcome), so they would not pass regardless of the fix.
9 Backdoors None. No magic values and no checks keyed on users, dates or hosts. The marker is a private type _GuardrailBlockedResponse(str), not a string match, so ordinary model text cannot fake a block.
10 Injection artifacts None in code, comments, docstrings or test data.

No deterministic risk flags were supplied with this request, so there were none to address one by one.

Code review

Approach. Returning the existing fixed blocked message instead of None fixes a real contract bug: chat()/achat() are typed to return str, and result.strip() crashed on None. Tagging that message with a private str subclass is a light way to mark it. There is no new public API, no new Agent parameter and no new config. agent.py grows by only +4/−1. This fits AGENTS.md (minimal, no new surface).

Things a maintainer should weigh before merging:

  1. Conflicts. mergeable_state=dirty and pipeline/blocked:conflict is set. The PR needs a rebase onto current main; its base 1aeee2f3 is behind, and recent main commits touched streaming/trace in start() (fix(trace): record streamed agent turns (start(stream=True)) #6087). Re-check the _start_stream and backend hunks after the rebase.
  2. Behaviour change for existing callers.
    • chat()/achat() now return text instead of None on input rejection. Downstream code that checked if response is None to detect a block will quietly see a string instead. This deserves a release-note line; isinstance(r, _GuardrailBlockedResponse) is private, so there is no public way to detect a block.
    • Caller-mode AutonomyResult now reports success=False when the input is blocked.
  3. Scope note: output blocks are affected too. _guardrail_blocked_message() is also used for output guardrail failures. With the new team check, output-blocked tasks are now marked failed and nothing is saved. This is probably the right behaviour, and the PR's own tests cover it, but it goes beyond "input guardrails" and is a team-semantics change.
  4. New early return in run_task/arun_task. if task.status == "failed": return is placed after execute_task. It fires for any path that sets failed inside execution, not only guardrail blocks. Please confirm no existing path sets failed and then expects the retry/completion-checker loop to run.
  5. The marker is fragile. Any str operation (.strip(), f-string, concatenation, str(x)) drops the subclass. It works today because every path in the diff returns the object untouched. Future wrapping in execute_task, hooks or output formatting would silently drop the block signal. A short comment at _process_task_result explaining the invariant would help.
  6. Double validation in planning. The initial prompt is validated in _start_with_planning, and each step prompt is then validated again by chat(). That is intended per the description. For LLM-based input validators it adds one call per step, which is acceptable.
  7. Minor. The lazy from ..guardrails.guardrail_result import _GuardrailBlockedResponse is repeated inside hot methods (start, astart, _outcome_for_result). It is cheap after the first import, so this is a nit.

Not verified: I did not run the tests; this is a fork PR and I am read-only. The pipeline labels show CI was not green on the head commit (blocked:ci). The PR body says the full suite was not run. A maintainer should run PYTHONPATH=. python -m pytest tests/ -x -q --timeout=30 after the rebase.

Verdict: Security PASS. The functional change is sound and small. Rebase it, confirm point 4, and note the None-to-string change in the changelog before merging.

P1 Empty responses stop retries

The new exit in run_task() also catches ordinary empty responses. _process_task_result() sets task.status to "failed" when an agent returns None or "", so a task with max_retries=3 now stops after its first empty response instead of trying again. The same issue affects arun_task().

Limit this early exit to guardrail rejection and keep the existing retry path for empty output.
