Add standard OpenTelemetry auto-discovery

Register the existing bundled instrumentor with opentelemetry_instrumentor, alongside its Phoenix entry point. The standard opentelemetry-instrument launcher can now activate Atomic Agents without application setup code.

• Accept additional launcher options without changing the existing provider/config arguments.
• Skip missing optional tracing dependencies at DEBUG for automatic loading, including the older 0.51b0 loader. Manual setup retains its import error and install hint.
• Keep both discovery paths on the same installation state. Document first-install-wins behavior, provider setup, privacy controls and disabling atomic_agents discovery.
• Add real-loader and enabled/disabled launcher subprocess tests. Add the launcher package to development dependencies and floor-test tooling only. Runtime dependencies and floors are unchanged.
• Refresh the affected codebase map documents.

LLM spans still need one provider instrumentor. This does not add automatic package installation through opentelemetry-bootstrap.

Verification

• Full core suite on the delivered branch: 658 passed, 3 skipped.
• Black and Flake8 across all four project directories: passed.
• Observability suite: 57 passed, including installed entry points, actual loader/launcher execution, missing-dependency continuation, privacy, duplicate prevention and teardown/reinstallation.
• Minimum telemetry stack: 57 passed with OpenTelemetry API/SDK 1.30.0, instrumentation 0.51b0, OpenInference 0.1.38 and semantic conventions 0.1.20.
• Regression controls exercised the original source and installed metadata before the fixes.

All checks used synthetic inputs, mocked model clients and local/no-op exporters. No live provider or remote backend calls. No version bump or release.
