docs: Update Linear MCP URL and connections content

Summary

• update the curated Linear MCP connection endpoint to https://mcp.linear.app/mcp, including generated setup surfaces like /integrations/linear
• expand the MCP connection docs with Vercel Connect setup, user/app-scoped auth, static credentials, filters, approvals, and troubleshooting
• expand the OpenAPI connection docs with base URL behavior, Vercel Connect usage, static credentials, filters, path parameter guidance, and approvals
• add a patch changeset for the generated setup and docs update
