Fix table overflow, add #tbl helper, minor cleanups

Changes

• Wrap long unbroken strings inside table cells instead of overflowing the margin.
• Set table text size to match #info notes (size-note).
• Add #tbl() helper for equal-width tables (cols: for count, columns: for custom widths).
• Set PDF title metadata via set document(title: title).
• Add a fallback font to idx-font.
• Use area-color in divider() instead of a hardcoded hex value.
• Rename heading_4_color to heading-4-color.
• Fix usage comment (index-doc.with → doc.with) and document #tbl.

Notes

• Heading, note, and code block layout are unchanged.
• table-font-size is currently unused and can be removed.
