docs: prescribe durable repository documentation

Summary

• make repository documentation an explicit durable-authority surface, not a task diary
• route rollout status, temporary inventories, implementation sequencing, discovery notes, and one-time checklists to issues, PRs, or ignored task state
• bound checked-in execution plans to durable coordination needs, explicit retirement, and lasting-value retention
• make the planner default to local task state only after an exact, shell-independent Git ignore check; unverified paths remain candidates rather than recommendations
• exclude .harness/task-state/ from later repository surveys so local execution state is survey-neutral
• dogfood and mechanically protect the rule with template-fitness anchors

• red-first regressions covered shell-special repository paths, fail-closed candidate/recommendation semantics, and survey idempotence
• node --test scripts/harness-bootstrap-plan.test.mjs — 273/273 pass
