Replace <sub> and <small> notes with ::: small divs, re-align the url row so the table parses as two columns with a real header, and fix a 'type:report' typo.
Grid-table alignment caused the earlier url-row breakage; a list table has no column edges to misalign.

The Citation Fields table on the citeable articles page had broken markup.
urlrow: One line was 3 characters too wide for its grid column. Pandoc then read the table as three columns. As a result, theurlrow was one cell that showed literal|characters, "Document Yaml" became an<h1>, and each row had an extra empty cell.<small>:<small>cannot contain a list.
