mainthat adds private load balancer support to the Metabase add-on form. When a cluster has a private load balancer provisioned, users can now front Metabase with the cluster's internal load balancer (reachable only from peered networks) instead of the public load balancer; selecting that option requires a custom domain since Porter does not provision anonporter.runhostname for privately-exposed add-ons.cc @d-g-town for review.
