After upgrading from a version with the former plugin ID, a title that kept a single-reference prefix reads as a reference the user wrote. The status then follows that PR/MR, labeled "from the session title", until a target is set. The migration section only mentioned the second prefix.

What does this PR do and why?
After upgrading from a version with the former plugin ID, a title that kept a single-reference prefix, such as
[#696] Review, reads as a reference the user wrote. Until a target is set, the status follows that PR/MR, labeled "from the session title". The migration section only mentioned the second prefix that setting a target adds, so this adds the status behavior.How to set up and validate locally
