Summary

Adds exhaustive unit test coverage for src/allowlist/index.ts using Vitest.
Tests isEnabled: false default passthrough.
Tests role: 'admin' administrative bypass.
Tests empty/null SQL validation.
Tests normalized SQL statement and AST comparisons.
Tests unlisted query rejection and tmp_allowlist_rejections audit logging.
Tests database RPC error resiliency.
