docs(plugin): add independent patch investigation and review

Summary

Add an independent investigation before a security patch and one focused review afterward.

Changes

• Require a read-only investigation of the security boundary and legitimate behavior.
• Give the reviewer the candidate diff without the patch rationale.
• Confirm review findings before changing the patch, then repeat relevant checks.
• Keep the existing patch workflow and provide a separate-pass fallback when delegation is unavailable.

Testing

• Full SDK suite on a local combination of all seven separate ports: 1,511 passed, 29 skipped, zero failures; randomized seed 1019194479.
• CLI skill tests: 25 passed.
• Formatting and whitespace checks passed.
• Reviewed the new instructions with the existing workflow.

Risk and rollout

Instruction-only change. No runtime APIs, dependencies, or release-version changes. The scope is separate from #527 and #583.

Public disclosure review

• No customer, partner, prospect, or user identities, data, or identifying details are included.
• No credentials, personal data, private source, scan findings, or nonpublic links or tickets are included.
• I reviewed the branch name, title, description, commits, changes, comments, logs, screenshots, attachments, and links for public disclosure.
