fix(env): a promote that changed nothing says "Nothing to promote" - #87

…d shows the integration note A real run whose answer is promoted: false (gateway 2026-10-21), or a no-op summary with no rollback anchor from an older gateway, prints "Nothing to promote — PROD already matches TEST.", no rollback anchor, exit 0.
