fix(readme): MPP mainnet overclaim, Node version, policy hook visibility, team disclosure - #203

- "Live on both Stellar networks" in the Networks section reads as covering
everything above it, including MPP - which has only ever settled on
testnet. Added an explicit carve-out, checked against the live
/api/v1/mpp/info on both boxes today.
- Node.js 18/20+ appeared in CONTRIBUTING.md, docs/quickstart-x402.md and
two examples' READMEs, while packages/sdk and packages/cli's own
package.json already require >=22.12.0 (what @stellar/stellar-sdk 16.x
actually needs). Unified all five.
- The pre-sign policy hook (initX402({ policy }), shipped in 0.16.0) only
showed up in packages/sdk/README.md - added a pointer to it from the
root README's feature list, since that's the first thing anyone reads.
- AgentLedger is cited as an independent third-party corroborator on the
OZ facilitator outage (Aug-Sep 2026) - accurate for when that happened,
but Fer (AgentLedger's founder) joined Nirium part-time in October 2026.
Added a dated note so a reader today has that context.
