docs: migrate to lean Northstar - #22

Move vision, architecture, contracts and vocabulary into docs/knowledge/ with one owner per topic. Fold the continuation handoff's operator rulings into their owning files, add a release contract from the 0.2.0 procedure, and record open questions. Replace roadmaps with docs/plan.md, promote the three triage notes, and record retired concepts. Remove .northstar/lifecycle/, docs/roadmaps/, docs/handoffs/ and docs/logs/. Switch the Queue manifest to v5 with queue closeout and no hooks. Rewrite the docs QA file lists, drop the vision index and roadmap heading checks, and point the token-count test at the styling contract. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

One-cut migration to lean Northstar (spec 042), following
northstar-lean/references/adopt.md. Drain was already true: nothing is in Queue for Nightfire.What changed
