Update code of conduct to require ai notices on communication

Claude posted a comment as me autonomously with no indication that it was bot-generated. I don't like this at all. This PR changes the code of conduct and CLAUDE.md in an effort to get it to not do this in future. You should know whether you're talking to me or an agent acting under my username.

It seems pretty innocuous, but because it's a change to the code of conduct I'd like sign-off from some more people. I think as a general policy it should require broad consensus to change the code of conduct. I'll tag it dev-meeting.

Yes, I didn't imagine this change would be controversial, I just don't like the idea that one person and one approver can change the code of conduct without others having the chance to weigh in.
