pickVerb only looked at the first and last leftover words, so a name like linear_list_recent_issues never conjugated and repeated Linear. A verb-less fallback also dropped its query clause. Long chips could not ellipsize because max-width:100% cycled against a shrink-wrapped max-content row.
The warning icon is aria-hidden, so a failed GitHub chip still read as a success. A visually-hidden Failed word puts failure in the accessible name. The 1Hz elapsed timer sat inside role=status, which is atomic, so the whole strip re-announced every second.

Tool-activity chips are layman sentences with tense, argument clauses, and an action glyph on a muted square for local tools. Known providers keep brand tiles. Status is Check / spinning CircleNotch / WarningCircle. Consecutive chips stack; they never fold into a count. DESIGN.md matches.
Tests, implementation, and docs are separate commits.
