fix(sdk): a plugin listed twice in one plugins list is refused at startup instead of keeping only its last entry - #1127
A plugin listed twice in one plugins list was silently reduced to its last entry. For example, SkilledOpenApiPlugin.init({ ...a }) and SkilledOpenApiPlugin.init({ ...b }) in one @App({ plugins }), or the same class twice. The first entry's options were dropped with no error and no warning.
Why it happened: PluginRegistry keys its entries by the plugin's provide token, which is the plugin class for the class form and for init(). A second entry with the same token replaced the first in the map. The key has been the token since the registry was first written. Every other registry keys by token too, and nothing in the plugin registry checked for a repeat.
Why refuse, and not install two instances:
The SDK has no plugin instance id. PluginMetadata.id exists, but options can't set name or id: init() keeps them as the plugin's own options, so options can't rename a plugin.
Two instances in one list would also register their option-derived providers under the same tokens in the app's registry. The last one would win for tools and context extensions, so one of the two would still be partly lost.
Refusing at startup follows what the SDK already does for apps that share an id (DuplicateAppIdError) and for a second install of a oncePerServer plugin (DuplicatePluginError).
What changed:
PluginRegistry now refuses a second entry with the same token in one list. It throws the new DuplicatePluginEntryError (code DUPLICATE_PLUGIN_ENTRY, a PublicMcpError with status 500, like DuplicatePluginError).
The message names the plugin, the list and the two entries by their place in it, then says how to fix it: list the plugin once with all its options, or install the other configuration on an @App of its own. The list is one of:
@FrontMcp({ plugins });
the plugins of app "<id>";
the plugins of agent "<id>";
the plugins of plugin "<name>", for a plugin's nested plugins.
To name the list, the check runs in the PluginRegistry constructor before super(), where the owner is known. It no longer runs in buildMap, which runs inside super(). A nested registry also gets the name of the plugin that hosts it.
The oncePerServer check still runs first. So a oncePerServer plugin listed twice in one list still fails with DuplicatePluginError and its reason.
DUPLICATE_PLUGIN_ENTRY joins the misconfiguration remedies. A Worker, Vercel or Lambda deployment that fails to start this way answers server_misconfigured with the code, like DUPLICATE_PLUGIN.
Unchanged: the same plugin in two apps, in the server's list and an app's, or in a plugin's nested plugins and its host's list. Each list has its own registry, so each one is a separate install with its own options.
Plugin entries the SDK adds from a server option. With the check above, such an entry and one the user lists would show up as "entries 1 and 2", pointing at an entry the user never wrote. I searched for every place the SDK adds a plugin entry (calls to init(), and pushes or spreads into a plugins list) across the SDK, react, testing, the CLI runtime and edge:
@FrontMcp({ observability }): scope.instance.ts adds ObservabilityPlugin.init(options) to the end of the server's list. A server that sets the option and also lists ObservabilityPlugin in @FrontMcp({ plugins }) now fails before the entry is added, with the new PluginConfiguredTwiceError (code PLUGIN_CONFIGURED_TWICE). Its message names the option and the entry, and doesn't pick one of them. The match is on the plugin's name, observability, as the frontmcp dev check below already does. So two copies of @frontmcp/observability (an ES-module project and the SDK's require) are caught too.
frontmcp dev's implied observability (FRONTMCP_DEV_EVENTS_FILE or FRONTMCP_DEV_EVENTS_IPC): it's added only when no list of the server or its apps installs ObservabilityPlugin, so it can't collide. Unchanged; it now shares the same listsObservabilityPlugin check.
createEdgeMcp({ managed }) in @frontmcp/edge adds SkilledOpenApiPlugin.init(...) to the config's plugins. That plugin is oncePerServer, so a user entry plus managed already fails with DuplicatePluginError. That error names no entries, but it doesn't name managed either. It's left for a follow-up, as libs/edge is being changed in a parallel PR.
No other SDK code adds a plugin entry.
User-facing change
What changed:
One plugins list (@FrontMcp, @App, @Agent or a plugin's nested plugins) that names a plugin twice now fails at startup with DuplicatePluginEntryError (code DUPLICATE_PLUGIN_ENTRY). This covers the class twice, two init() calls, and the class plus an init().
A server that sets @FrontMcp({ observability }) and also lists ObservabilityPlugin in @FrontMcp({ plugins }) fails at startup with PluginConfiguredTwiceError (code PLUGIN_CONFIGURED_TWICE).
Both errors are exported from @frontmcp/sdk.
Before → after:
A plugin listed twice:
Before: the server started with only the last entry installed. The other entry's options were gone without a word.
After: Plugin "ledger" is listed twice in the plugins of app "billing" (entries 1 and 3), and only one entry could be installed. List it once, with all its options in that entry, or install the other configuration on an @App of its own.
observability set and ObservabilityPlugin listed:
Before: the server started. With one copy of the package, the plugin observability adds replaced the listed one, so the entry's options were dropped.
After: ObservabilityPlugin is configured twice: by @FrontMcp({ observability }) and by an ObservabilityPlugin entry in @FrontMcp({ plugins }). Configure it in one place: move the entry's options to observability and remove the entry, or remove observability.
A oncePerServer plugin listed twice still gets DuplicatePluginError.
Two apps, or the server and an app, may each still install the same plugin with their own options. observability alone, or an ObservabilityPlugin entry alone, still starts.
frontmcp.dev pages affected (if known):
the plugins guide (installing plugins, and installing one plugin with two configurations);
the observability setup page (the option or the plugin, not both);
the error codes reference (new DUPLICATE_PLUGIN_ENTRY and PLUGIN_CONFIGURED_TWICE);
the Cloudflare deployment page's list of server_misconfigured codes.
Skills catalog entries updated:
frontmcp-development/references/create-plugin.md: a new "A plugin listed twice in one plugins list" section. It covers the error and how it names the list, the 1.11.0 behavior, how to run two configurations on two apps, and the case of a server option that installs the plugin. The old sentence saying the last entry is installed is removed.
frontmcp-development/references/official-plugins.md: list each plugin once per plugins list, and what happens otherwise.
frontmcp-observability/SKILL.md: observability installs the plugin, so don't also list it; the error, and how to fix it; a troubleshooting row for PLUGIN_CONFIGURED_TWICE.
frontmcp-deployment/references/deploy-to-cloudflare.md: DUPLICATE_PLUGIN_ENTRY and PLUGIN_CONFIGURED_TWICE added to the server_misconfigured codes.
The @frontmcp/observability README says to use the option or the plugin, not both.
Tests
Added:
libs/sdk/src/plugin/__tests__/plugin.registry.duplicate-entry.spec.ts (8 tests, all on a real server through FrontMcpInstance.createDirect):
the plugin class twice in @FrontMcp({ plugins }) is refused, naming that list and entries 1 and 2;
two init() entries in an @App list with another plugin between them are refused, naming the plugins of app "billing" and entries 1 and 3;
the class plus an init() is refused;
a plugin's nested plugins listing a plugin twice is refused, naming the plugins of plugin "host";
the error carries DUPLICATE_PLUGIN_ENTRY and the fix;
a oncePerServer plugin listed twice still gets DuplicatePluginError, not the new error;
two apps each install the plugin, and each app's tool resolves its own options;
the server and an app each install the plugin, and the app's tool resolves the app's options.
libs/sdk/src/scope/__tests__/observability-configured-twice.spec.ts (3 tests, with an ObservabilityPlugin stand-in registered through registerOptionalModule):
observability plus an ObservabilityPlugin entry fails with PluginConfiguredTwiceError and its message, not with DuplicatePluginEntryError;
observability alone starts and installs the plugin;
the entry alone starts and installs the plugin.
plugin.registry.once-per-server.spec.ts: the test that expected a plugin without oncePerServer listed twice to start now expects DuplicatePluginEntryError, and not DuplicatePluginError.
misconfiguration.spec.ts: DUPLICATE_PLUGIN_ENTRY and PLUGIN_CONFIGURED_TWICE map to their remedies.
Without the registry check, the refusal tests in the duplicate-entry spec fail. Without the scope guard, the first observability test fails: the error is DuplicatePluginEntryError with entries 1 and 2.
Bundle size: the two errors and their remedies, and the list-naming check, add 1,578 bytes to create() + one tool (1,289,009 → 1,290,587). They add 1,581 bytes to create() + WebMcpPlugin + one tool (1,296,199 → 1,297,780), and 1,069 bytes to the initial chunk of a Vite app (763,249 → 764,318). The budgets are set to these measured sizes. DuplicatePluginEntryError and PluginConfiguredTwiceError are added to the pinned root exports.
No e2e was added. No existing e2e app covers plugin composition: demo-e2e-multiapp installs no plugins, and demo-e2e-hooks uses plugins for hook order. The unit tests above start a real server.
Run:
npx nx run-many -t build -p sdk,testing: pass.
npx nx test sdk: 693 suites, 9656 tests. 9655 pass. One timing test failed once under load: scope-init-perf.spec.ts, "parseFrontMcpConfigLite should not be dramatically slower than full parse", a config-parse micro-benchmark this PR doesn't touch. It passed 3 of 3 runs on its own (9 tests each).
npx nx run-many -t test -p 'plugin-*': 9 projects, 217 suites, 3804 tests, all pass. That includes plugin-skilled-openapi (51 suites, 753 tests), so no plugin of ours lists a plugin twice.
npx nx test skills: 3 suites, 249 tests, all pass.
npx nx test demo-e2e-bundle-size: 12 suites, 63 tests, all pass with the new budgets.
npx nx run sdk:typecheck and npx tsc --noEmit -p libs/sdk/tsconfig.lib.json: pass.
npx eslint and npx prettier --check on the changed files, and node scripts/fix-unused-imports.mjs: clean.
Summary by CodeRabbit
New Features
Startup errors now identify when a plugin appears more than once in the same plugin list, including the conflicting entries.
A separate error identifies when observability is configured both through the observability setting and as a plugin.
Bug Fixes
Duplicate plugin entries in a single list now cause startup to fail instead of silently keeping the last entry. Separate app installations can still use the same plugin independently.
Documentation
Updated setup and troubleshooting guidance to explain duplicate plugin errors, their distinctions, and how to resolve them.
…instead of losing its first entry
A plugins list keeps one entry per plugin token, so two entries of one plugin (the class twice, two init()
calls, or both forms) left only the last one installed, and the first entry's options were dropped without a
word. The SDK has no plugin instance id to install two instances from one list: options can't rename a plugin,
and two instances would register their providers under the same tokens.
Startup now fails with DuplicatePluginEntryError (code DUPLICATE_PLUGIN_ENTRY), naming the plugin and both
entries by their place in the list, and saying to list it once or install the other configuration on an app of
its own. The same plugin in two apps, or in the server's list and an app's, is still two installs. A
oncePerServer plugin listed twice keeps its DuplicatePluginError.
The browser bundle budgets take the error and its misconfiguration remedy: 642 bytes for create() + one tool
and for create() + WebMcpPlugin + one tool, 535 for the initial chunk of a Vite app.
… an error naming both, and a duplicate entry names its list
@FrontMcp({ observability }) adds its own ObservabilityPlugin to the end of the server's plugins list. With an
ObservabilityPlugin entry there too, startup failed with DuplicatePluginEntryError naming entries 1 and 2, one of
which the user never wrote. The server now checks before adding it and fails with PluginConfiguredTwiceError (code
PLUGIN_CONFIGURED_TWICE), naming the option and the entry and saying to configure the plugin in one place. The match
is on the plugin's name, as frontmcp dev's implied observability already does, so a second copy of the package is
caught too. It is the only plugin entry the SDK adds from a server option.
DuplicatePluginEntryError now names the list that holds the duplicate: @FrontMcp({ plugins }), an app's, an agent's,
or a plugin's nested plugins. The check runs in the PluginRegistry constructor before super(), where the owner is
known, and a nested registry gets the name of the plugin that hosts it.
The bundle budgets take both errors, their remedies and the check: 1,578 bytes in all for create() + one tool,
1,581 with WebMcpPlugin, and 1,069 for the initial chunk of a Vite app.
No actionable comments were generated in the recent review.
Walkthrough
The SDK now rejects repeated plugin entries in one plugin list and rejects observability configured through both a server option and the plugins list. The changes add error classes and remedies, update tests and documentation, and increase browser bundle-size budgets.
Adds DuplicatePluginEntryError and PluginConfiguredTwiceError, with remedies and classification tests. The SDK root export fixture includes both classes, the browser bundle-size budgets increase, and the deployment reference lists both error codes.
PluginRegistry rejects repeated entries in a plugin list and reports the list and entry positions. oncePerServer duplicates retain their existing error. Tests and documentation cover duplicate entries and separate app installations.
Scope.initialize rejects observability configured through both observability and the server plugins list. Development-events observability checks now use listsObservabilityPlugin and return no configuration when neither development event source is enabled. Tests and documentation describe the configuration paths and conflict.
agentfront/frontmcp#1013: Defines when development events imply observability and suppresses that inference when ObservabilityPlugin is installed. It uses the observability configuration path involved in this change.

…ames is refused too
The duplicate-entry check skipped oncePerServer entries and left them to the once-per-server claim, which keys by name. Two entries of one class with different inline names passed both, and the second replaced the first. Only a second entry under the same name is left to DuplicatePluginError now.

What and why
Stacks on #1123.
A plugin listed twice in one
pluginslist was silently reduced to its last entry. For example,SkilledOpenApiPlugin.init({ ...a })andSkilledOpenApiPlugin.init({ ...b })in one@App({ plugins }), or the same class twice. The first entry's options were dropped with no error and no warning.Why it happened:
PluginRegistrykeys its entries by the plugin'sprovidetoken, which is the plugin class for the class form and forinit(). A second entry with the same token replaced the first in the map. The key has been the token since the registry was first written. Every other registry keys by token too, and nothing in the plugin registry checked for a repeat.Why refuse, and not install two instances:
PluginMetadata.idexists, but options can't setnameorid:init()keeps them as the plugin's own options, so options can't rename a plugin.DuplicateAppIdError) and for a second install of aoncePerServerplugin (DuplicatePluginError).What changed:
PluginRegistrynow refuses a second entry with the same token in one list. It throws the newDuplicatePluginEntryError(codeDUPLICATE_PLUGIN_ENTRY, aPublicMcpErrorwith status 500, likeDuplicatePluginError).@Appof its own. The list is one of:@FrontMcp({ plugins });the plugins of app "<id>";the plugins of agent "<id>";the plugins of plugin "<name>", for a plugin's nestedplugins.PluginRegistryconstructor beforesuper(), where the owner is known. It no longer runs inbuildMap, which runs insidesuper(). A nested registry also gets the name of the plugin that hosts it.oncePerServercheck still runs first. So aoncePerServerplugin listed twice in one list still fails withDuplicatePluginErrorand its reason.DUPLICATE_PLUGIN_ENTRYjoins the misconfiguration remedies. A Worker, Vercel or Lambda deployment that fails to start this way answersserver_misconfiguredwith the code, likeDUPLICATE_PLUGIN.pluginsand its host's list. Each list has its own registry, so each one is a separate install with its own options.Plugin entries the SDK adds from a server option. With the check above, such an entry and one the user lists would show up as "entries 1 and 2", pointing at an entry the user never wrote. I searched for every place the SDK adds a plugin entry (calls to
init(), and pushes or spreads into a plugins list) across the SDK, react, testing, the CLI runtime and edge:@FrontMcp({ observability }):scope.instance.tsaddsObservabilityPlugin.init(options)to the end of the server's list. A server that sets the option and also listsObservabilityPluginin@FrontMcp({ plugins })now fails before the entry is added, with the newPluginConfiguredTwiceError(codePLUGIN_CONFIGURED_TWICE). Its message names the option and the entry, and doesn't pick one of them. The match is on the plugin's name,observability, as thefrontmcp devcheck below already does. So two copies of@frontmcp/observability(an ES-module project and the SDK'srequire) are caught too.frontmcp dev's implied observability (FRONTMCP_DEV_EVENTS_FILEorFRONTMCP_DEV_EVENTS_IPC): it's added only when no list of the server or its apps installsObservabilityPlugin, so it can't collide. Unchanged; it now shares the samelistsObservabilityPlugincheck.createEdgeMcp({ managed })in@frontmcp/edgeaddsSkilledOpenApiPlugin.init(...)to the config's plugins. That plugin isoncePerServer, so a user entry plusmanagedalready fails withDuplicatePluginError. That error names no entries, but it doesn't namemanagedeither. It's left for a follow-up, aslibs/edgeis being changed in a parallel PR.No other SDK code adds a plugin entry.
User-facing change
pluginslist (@FrontMcp,@App,@Agentor a plugin's nestedplugins) that names a plugin twice now fails at startup withDuplicatePluginEntryError(codeDUPLICATE_PLUGIN_ENTRY). This covers the class twice, twoinit()calls, and the class plus aninit().@FrontMcp({ observability })and also listsObservabilityPluginin@FrontMcp({ plugins })fails at startup withPluginConfiguredTwiceError(codePLUGIN_CONFIGURED_TWICE).@frontmcp/sdk.Plugin "ledger" is listed twice in the plugins of app "billing" (entries 1 and 3), and only one entry could be installed. List it once, with all its options in that entry, or install the other configuration on an @App of its own.observabilityset andObservabilityPluginlisted:observabilityadds replaced the listed one, so the entry's options were dropped.ObservabilityPlugin is configured twice: by @FrontMcp({ observability }) and by an ObservabilityPlugin entry in @FrontMcp({ plugins }).
