What does this PR do?

When a generate POST returns nothing_to_start, another client can accept the staged batch and start generation before the next GET. Both no-op branches previously returned that GET unconditionally, so an active proposing state exited 0 and text output claimed that zero proposals were staged.

All trigger outcomes now go through the same observed-state loop. Active generation keeps waiting, including idle visibility gaps after proposals start; failures and the shared deadline retain their existing outcomes. An immediate idle read after a no-op still returns the current batch (including an empty batch already accepted elsewhere). The existing guards against starting proposals twice remain in force.

Validation

Local macOS, Node v24.19.0, based on main 988c0355bb05631e9c6c1785e443937ff2a88c0e:

  • 19 new regression/control tests; 17 failed before the fix. Both plan test files now pass: 108 tests.
  • npm run test:coverage: 4,605 passed, 2 skipped. Coverage: statements 89.69%, branches 84.79%, functions 83.17%, lines 91.91%.
  • npm run test:e2e: 103 passed, 1 skipped.
  • Six additional full CLI subprocess checks passed, with offline HTTP fixtures: existing staged batch, initial no-op race in JSON/text, accepted-stage and 409-attach controls, and failed-generation control. The race now reads the completed new batch instead of exiting on the first active snapshot.
  • Build, typecheck, lint, and format checks passed. Lint reports 178 existing warnings and no errors.

These checks use simulated API responses and local fixture services; they do not claim a hosted concurrency observation or remote CI approval.

Checklist

  • PR targets the main branch.
  • Commits follow Conventional Commits.
  • npm run lint and npm run format:check pass.
  • npm run typecheck passes.
  • Tests pass and coverage stays above the 80% gate.
  • New behavior is covered by mock-based unit tests.
  • No secrets, API keys, internal endpoints, or personal data are included.
  • User-facing behavior is documented in DOCUMENTATION.md.

Notes for reviewers

The empty-idle no-op outcome is deliberately retained for compatibility. The baseline post-accept race test already expects runPlanGenerate to return normally with an empty batch after nothing_to_start. The separate renderer comment about never settling empty applies after the proposals rung has been started/observed, not to that immediate no-op case. DOCUMENTATION.md now explicitly distinguishes these outcomes. Full CLI checks against both baseline and this branch confirm that acceptance by another client returns idle/empty with exit 0 and exactly one CLI POST; an observed active regeneration must still be awaited. Changing the established post-accept exit code is outside this fix.

Review follow-up validation: the 108 plan tests, build, typecheck, lint and formatting checks passed again. Eight full CLI fixture scenarios passed on the updated branch, plus two JSON/text post-accept controls on the baseline. The follow-up only adds comments/documentation; stripping comments produces identical executable code.

Issue #516 is still awaiting triage and assignment. This PR is being submitted before assignment; the repository's issue-assignment gate remains outstanding. No claim or maintainer approval is implied.

Summary by CodeRabbit

Summary by CodeRabbit

  • Bug Fixes
    • Plan generation now checks status after a trigger reports that there is nothing to start. If generation is active, the command waits for it to settle; if no generation is active and no proposals are staged, it exits with the current empty result.
    • After observing active generation, the command continues checking through empty idle reads until proposals appear, generation fails, or the wait limit is reached. It also detects proposals staged after an idle gap.

Walkthrough

When a trigger returns nothing_to_start, the polling loop reads generation status instead of settling based on the trigger result. Active observations keep polling. An eligible idle observation can settle the run. Tests cover initial and later trigger responses, and documentation describes concurrent generation behavior.

doTrigger no longer returns a trigger outcome for ladderLoop to use for settlement. The loop continues to read status after no-op responses. Full details: Linked Issues check

Explanation

Issue #516 coding requirements are met. The implementation routes both initial and later nothing_to_start results through the observed-state polling loop. Active states remain pending, including visibility gaps before proposals appear. Failure, timeout, and interruption handling remain covered by the existing logic and regression tests. The tests cover JSON and text output, immediate idle results, trigger and read counts, and the no-duplicate-proposals guard. No extra generate POST occurs for an empty proposal list.

Full details: Out of Scope Changes check

Explanation

The changes remain within issue #516. The source change updates no-op handling during plan generation. The added tests verify active polling, idle and failure outcomes, timeout behavior, output, and duplicate-POST protection. The documentation change describes the same concurrent-generation behavior. No unrelated product change is shown.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Do not settle an empty no-op read as success.

When nothing_to_start is followed by an idle read with no proposals, state.noopSettlesIdle makes runGenerationLadder return normally. runPlanGenerate then prints the normal result and exits successfully with proposals: []. This reports completion even though the CLI neither observed an active generation nor received a staged batch.

The zero-proposal renderer is documented as unreachable from the ladder, and no applicable exception defines this race as successful. Keep the no-duplicate guard, but return an explicit non-success outcome for this state.

Thanks @luckyzd. The change is well scoped and addresses #516. Plan generation will be removed from the CLI in a future release, so a fix here would go away with the command. Closing without merging; this isn't a comment on the quality of the work. Details are on #516.
