Persist submitted transaction hashes with operation, round, network, and expiry metadata before RPC submission. Reconcile pending hashes before rebuilding operations; confirmed transactions are checkpointed, definitive failures stop retries, and expired transactions receive one replacement. Adds fake-clock and fake-RPC coverage.
