fix: limit doctor recursion check to MCP servers

Summary

• parse Codex config and inspect only the mcp_servers table for recursive bridge registration
• avoid false positives from ordinary project trust paths containing codex-app-mcp
• add focused regression coverage and prepare v0.1.1 metadata

Validation

• 148 non-live tests passed (4 live deselected)
• Ruff lint and format checks passed
• sdist and wheel build passed
• betterleaks working-tree scan found no leaks
• doctor completed with ok: true against a config containing the repository trust entry
