+Exclude self-employed health insurance premiums already deducted above the line from federal and applicable state itemized medical expenses, cap New Jersey dependent exclusions per payer, preserve North Dakota and New Mexico gross medical costs and caller-supplied subtotals by restoring only exclusions applied by the federal formula, and exclude already-deducted premiums from Missouri's qualifying pool using its required whole-percent overlap ratio. Require policyengine-core>=3.32.27 so deleting and recalculating a supplied medical subtotal also preserves correct North Dakota and New Mexico results.
