fix(host-service): bound gh fan-out and drop queued calls when the gate opens - #9193
On a host with many projects, the PR refresh makes "GitHub unreachable" flicker: a warning every few minutes, PR links stop updating, then recover. Fixes#8998.
Root cause
refreshEligibleProjects fans out with Promise.all over every project, then over every tracked head ref, then over every matched PR — several gh processes each. Nothing capped how many ran at once, so a ~100-project host started hundreds of children in one tick. Each child is killed by exec-gh.ts's 10s timeout, and isGitHubUnreachableError counted killed && signal as GitHub being down. That opened a 60s hold and logged "GitHub unreachable"; a success from the same burst closed it again, so the warning flickered, and the hold's recovery refresh started the next burst.
Fix
GhCallLimiter (4 slots, FIFO) wraps the injected execGh under ConditionalGh. A queued call waits for a slot instead of spending its 10s timeout; the wrapper's parsing and cache bookkeeping stay outside the slot.
An exec kill moves to isGitHubTimeoutError. It no longer opens the gate on its own, and still skips the Octokit fallback (that would add work to the same overload). Three consecutive kills with no answer between open unreachable — what a dropped network looks like.
The gate is checked before a lookup queues, so opening a hold now drains the queue: already-queued lookups reject with the hold instead of spawning. Explicit probes still run.
Real transport failures keep opening the hold through their codes and gh's stderr.
On reuse: the repo has no p-limit/p-queue dependency. mapWithConcurrency in git/utils/git-helpers.ts bounds a fixed array of work, which this is not — these calls arrive from five call sites and the queue has to be cancelled when the gate closes, so it needs drain().
How tested
bun test packages/host-service/src/runtime/pull-requests — 120 pass.
Ten concurrent sweeps peak at 4 in-flight spawns.
Six lookups queued behind four held slots: opening the hold leaves the spawn count at 4 and none of the six ever runs (10 without the drain).
One SIGTERM kill leaves getGithubStatus() null; a third opens the hold; an ENOTFOUND opens it immediately.
bun run --cwd packages/host-service typecheck clean; ./scripts/lint.sh clean. Full host-service suite: 2707 pass, 3 fail. All three are test/integration/terminal.integration.test.ts daemon cases and they fail identically on the origin/main baseline 170e1f4ad7 in this environment (measured: checked out that commit's packages/host-service/src, same 3 red, 6 pass).
Tradeoffs
Foreground latency is new. Before this change a user-triggered refresh spawned gh immediately; now it enters the same FIFO queue as the background sweep and can wait for the calls already ahead of it. Bounded by the 4 slots, but it is a change, not a pre-existing cost. A priority lane (as GitStatusRefreshLimiter does for git status) is the follow-up if it bites.
github-search and workspace-creation still call execGh directly, so they stay unbounded. That is unchanged by this PR, and is the next place the same cap should go.
Summary by CodeRabbit
Bug Fixes
GitHub requests are limited to four concurrent calls.
When GitHub becomes unavailable, queued background requests are stopped while explicit probes remain queued.
Timeouts are distinguished from connection failures. Three consecutive timeouts can trigger a temporary pause; successful requests reset the timeout count.
Unavailable and timed-out requests no longer fall back to Octokit.

Walkthrough
The pull-request runtime now limits concurrent GitHub calls and distinguishes timeout errors from other failures. Three consecutive timeouts open an unreachable hold. When a hold opens, the runtime drains queued background calls and retains queued probes.

Summary
Timeout classification and availability gate packages/host-service/src/runtime/pull-requests/utils/github-availability/*
Timeout errors are classified separately from unreachable errors. The availability gate opens an unreachable hold after three consecutive timeouts and resets the timeout count after success or a non-timeout failure.
GhCallLimiter limits active calls, queues excess calls in FIFO order, and rejects queued background calls when drained. It retains queued probes. Tests cover concurrency limits, failures, and draining.
Runtime integration and coverage packages/host-service/src/runtime/pull-requests/pull-requests.ts, packages/host-service/src/runtime/pull-requests/pull-requests.test.ts
The runtime uses the limiter for GitHub calls. Timeout and unreachable errors skip Octokit fallback, and opening a hold drains queued calls. Tests cover concurrency limits, hold behavior, and queued probes.
Explicit refreshes can use more GitHub API quota than before. Restore conditional requests for probes; the remaining risk is bounded and does not block merging.

Explanation
Issue #8998 requires PR status to keep loading and the warning to appear only for actual GitHub failures. The PR routes PR-runtime gh calls through GhCallLimiter with four slots, classifies killed executions as timeouts, and opens the unreachable hold only after three consecutive kills. It drains queued background calls when the hold opens and preserves explicit probes. Tests cover the concurrency limit, queue draining, timeout handling, and transport failures such as ENOTFOUND.

### Issue 1
packages/host-service/src/runtime/pull-requests/utils/github-availability/github-availability.ts:246-248
`recordFailure()` skips `countTimeout()` for classified failures, so an auth or rate-limit response does not reset the timeout count. Two kills followed by such a response leave `timeouts` at two. After that hold expires, one isolated kill opens a new `unreachable` hold and drains all queued lookups across the host.
That is not three kills with no answer between. Reset the timeout count when a non-timeout response is recorded, including classified responses.
### Issue 2
packages/host-service/src/runtime/pull-requests/utils/gh-concurrency/gh-concurrency.ts:58-59
`release()` starts the next queued call before `fetchFromGitHub()` sees the failure and opens the hold. With four running calls and more waiting, a transport failure starts another `gh` child before `drain()` runs. The revised test even expects five attempts. This spends resources on a network already known to be unavailable.
Let the failure reach the gate before handing off the slot. Test draining through a rejected call rather than calling `noteGitHubFailure()` directly.
### Issue 3
packages/host-service/src/runtime/pull-requests/pull-requests.ts:1329
`drain()` also drops queued explicit probes. If a user refresh waits behind a sweep and a running call opens the hold, the probe receives `GitHubUnavailableError` without ever reaching GitHub. It cannot restore PR updates early, even if the network has recovered.
Preserve explicit probes when draining background lookups, without bypassing the concurrency limit.

For each issue above, determine whether it is valid and should be fixed. If so, fix it directly.
Summary

…te opens
Every PR refresh fanned out over every project, branch, and PR detail at
once, so a host with ~100 projects started hundreds of `gh` children in
the same tick. Each child then raced its 10s execFile deadline, and
`isGitHubUnreachableError` counted every one of those kills as GitHub
being down. The gate opened a 60s hold and logged "GitHub unreachable";
a success from the same burst closed it again, so the warning flickered
and the recovery sweep started the next burst. FixesParadevAI#8998.
Spawns now go through a 4-slot FIFO around the injected `execGh`, under
`ConditionalGh`, so a queued call does not spend its timeout while it
waits. An exec kill moves to `isGitHubTimeoutError` and no longer opens
the gate on its own; three kills with no answer between still do, and
any classified answer (auth, rate limit, transport) restarts that count.
Octokit is still skipped for a kill, since it would add work to the same
overload. Real transport failures keep opening the hold through their
codes and gh's stderr.
The gate is checked before a call queues, so opening a hold now drains
the queue; the limiter reports each failure to the gate before it hands
the slot on, so the drain lands ahead of the next spawn. Explicit probes
are tagged in that queue: a hold drops background waiters but keeps a
probe in place, so a user refresh still gets its turn and can reopen the
gate early, without bypassing the slots.
Proved with `bun test packages/host-service/src/runtime/pull-requests`
(122 pass): 10 concurrent sweeps peak at 4 in-flight spawns; six lookups
queued behind four held slots leave the spawn count at 4 when one of them
fails and opens the hold (10 without the drain, 5 if the slot moves
before the gate sees the failure); three probes queued behind a sweep all
run across two holds; a repeat kill after a rate-limit answer opens
nothing; one SIGTERM kill leaves `getGitHubStatus()` null while a third
opens the hold; an `ENOTFOUND` opens it immediately.

Keep ETag revalidation on probe calls.
bypassCache skips the manager’s TTL cache, but it does not require bypassing ConditionalGh. The raw probe lane omits If-None-Match, so eligible REST GET refreshes can consume quota when the same requests could return 304. Wrap the probe lane in its own ConditionalGh; it still performs a real request, so availability probes continue to work.
