Added another error page for when a user select co-admin or same vax different person options - and can't continue because the site has been deactivated just now.

@benjystanton I've added the extra error page we discussed earlier, updated the index page with the new error page, and also added a para of text below the H1 on the Add batch page. No rush as Ileri already has the screenshot she needs for the error page. |

Created new error page (and added it to prototype index) for scenario where a user opts for co-admin or same vax different patient, and the site has just been deactivated.
