Make Infection a real quality gate instead of an informational run.
The latest successful run reports 100% mutation code coverage and 78% covered-code MSI. This starts both
min-msiandmin-covered-msiat 75%, leaving a small buffer while preventing silent regression to an ineffective mutation suite.Follow-up work to simplify and strengthen the broader test/quality stack is tracked in #81.
