Use stub active span as fallback (ctx.otelFallbackSpan), rebind it while inside GalaTransaction/authorize/handle/afterTransaction so children stay nested when AsyncLocalStorage is dropped. Co-authored-by: Cursor <cursoragent@cursor.com>
