Publish the Instructor range update merged in #310, on top of the released 2.12.1 fixes. No additional production behavior changes in this PR.

• Set the package version to 2.13.0 and update the lockfile.
• Finalize the release notes, including the changed retry-count semantics and the 1.14.5 compatibility boundary.
• Refresh the map's package version and real-transport test description.

• Integrated core suite: 647 passed, 3 credential-gated skips.
• Minimum direct runtime dependencies: 647 passed, 3 credential-gated skips on Python 3.12.
• Six real-Instructor tests with mocked HTTP transport cover all execution paths and strict streaming settings.
• Black and Flake8 pass across all four directories.
• Distribution validation and clean-wheel installation are checked before publishing. No live model calls.
