Fix URL rewriting in GFM export for bracketed link labels and linked images. For example, in Octavo we want to rewrite
[Core [API]](core.html)'score.htmltocore.md. Currently it stays unchanged even when the link callback requestscore.md, leaving exported Markdown pointing to HTML pages.
