What does this PR do?

Honors PostgreSQL schema configuration, adds exact transaction amount/reference fields with migrations, validates SEP-10 challenge envelopes, and gives webhook callbacks cooperative cancellation on timeout and shutdown.

How to test?

Run bun test tests/runtime/sql-adapter-interactive-tx.test.ts tests/runtime/sql-adapter-migrations.test.ts tests/runtime/postgres-schema.integration.test.ts tests/runtime/webhook-processor.unit.test.ts tests/mvp-express.integration.test.ts.

Expected: 46 passing and 1 skipped without PostgreSQL. Set ANCHOR_KIT_TEST_POSTGRES_URL to a disposable PostgreSQL database to run the schema-isolation integration check. See docs/mvp-express.md for commands and details.
