Add Icon and icons on Implementation, ToolDefinition, Resource, ResourceTemplate, Prompt, ResourceLink.
Annotations on resources and content blocks. _meta on content blocks. Implementation.websiteUrl and
description. PromptReference.title. ResourceLink.title and size. Add builder methods in the server DSL for the
new metadata and schema test cases.
