• An armed Lavish source can end up with no runner, and nothing says so - captain's answers had no collector

HelloWorldSungin/firstmate#268 · Filter by author HelloWorldSungin opened on Sep 5, 2026

• Captain-answer attribution has no per-key record, so the gated-hold shape cannot be marked

HelloWorldSungin/firstmate#263 · Filter by author HelloWorldSungin opened on Sep 4, 2026

• run_watcher_bounded's fixed 10s wall produces reds that look like regressions in unrelated changes

HelloWorldSungin/firstmate#262 · Filter by author HelloWorldSungin opened on Sep 3, 2026

• A validation run can carry a different project's intent, silently corrupting scope verdicts and the PR body

• The merge argument allow-list duplicates gh-axi's parser to decide safety, and that model has been wrong twice

HelloWorldSungin/firstmate#259 · Filter by author HelloWorldSungin opened on Sep 3, 2026

• Guarded merge enforces the default-branch target rule on GitHub only, not GitLab

HelloWorldSungin/firstmate#257 · Filter by author HelloWorldSungin opened on Sep 3, 2026

• real-herdr-gated carries only a 1.41x per-script timeout margin, accepted rather than fixed

HelloWorldSungin/firstmate#256 · Filter by author HelloWorldSungin opened on Sep 3, 2026

• gh-axi accepts only owner/name for --repo, so the guarded merge cannot pin its forge host

HelloWorldSungin/firstmate#255 · Filter by author HelloWorldSungin opened on Sep 3, 2026

• fm-watch-arm's signal handler waits on its child unbounded, so a signalled arm can hang and orphans its watcher when killed

HelloWorldSungin/firstmate#251 · Filter by author HelloWorldSungin opened on Sep 3, 2026

• Fleet blocked: the no-mistakes gate agent hangs alive-and-idle on silent API reads, so no task can pass its test step

• Text delivery through fm-send.sh reaches the same wrong-task exposure #218 closed on the interrupt path

HelloWorldSungin/firstmate#247 · Filter by author HelloWorldSungin opened on Sep 3, 2026

• A test fixture wrote user.name=Test into the real repository git config, so every crew commit was authored as Test <test@example.com>

HelloWorldSungin/firstmate#245 · Filter by author HelloWorldSungin opened on Sep 2, 2026

• The pipeline attestation is never rebound after the pr step, so any task with later fix rounds ends with a permanently stale body

• The fixer resolves an instruction-versus-assertion conflict by rewriting the assertion, silently editing the rule it contradicts

• Stale-base merge guard dropped as undecidable: all three founding cases merge cleanly, and staleness cannot imply intent

HelloWorldSungin/firstmate#239 · Filter by author HelloWorldSungin opened on Sep 2, 2026

• A reviewer finding that says 'amend or squash' instructs the exact rewrite the custody guard then fails the run for

HelloWorldSungin/firstmate#238 · Filter by author HelloWorldSungin opened on Sep 2, 2026

• Re-running the attestation check can never clear it: the re-run replays a stale PR body from the cached event payload

• The ci auto-fixer converts a transient attestation failure into unreviewed commits that change reviewed behaviour

HelloWorldSungin/firstmate#236 · Filter by author HelloWorldSungin opened on Sep 2, 2026

• A steer delegated ask-user finding authority back to the implementation worker, and nothing in the fleet noticed for four findings

• Stale-base guard: close the GitHub queue-transition race with an atomic mutation boundary instead of a fourth preflight

HelloWorldSungin/firstmate#224 · Filter by author HelloWorldSungin opened on Aug 31, 2026

• A steer to an idle agy worker is reported delivered but never reaches the agent, and the validation trigger closes the blocker anyway
